Abstract
背景:
台灣即將邁入超高齡社會,高齡人口的非傳染性疾病(non-communicable diseases, NCDs)負擔日益加重。傳統的橫斷式健康指標無法完整揭示老年人在長期健康變化上的異質性,因此本研究透過縱向分析方法,探討台灣老年人口NCDs相關疾病負擔的長期發展軌跡,以支持未來健康老化政策與資源配置。
研究方法:
本研究利用2000年至2022年間的全民健康保險研究資料庫(National Health Insurance Research Database, NHIRD),觀察65歲以上個案的歷年健康資料,結合非傳染性病診斷紀錄、嚴重程度分類標準與全球疾病負擔(Global Burden of Disease, GBD)訂定的健康損失權重(Disability Weights, DW),計算每位個案逐年之共病校正健康生命損失年(Years Lived with Disability, YLD)指標。並進一步採用群組化軌跡模型(Group-Based Trajectory Modeling, GBTM),分析台灣老年人口YLD軌跡之潛在異質分組,並探討其社會人口學因素、共病組合與死亡風險的關聯性。
研究結果:
5,098,280名觀察個案經GBTM歸納出四組YLD發展軌跡,分別為「穩定低負擔組」(20.38%)、「低負擔上升組」(32.82%)、「中負擔上升組」(37.10%)與「早期高負擔組」(9.69%)。不同四組軌跡的人們在老年承擔不同程度的健康損失負擔之外,「穩定低負擔組」的增加幅度也明顯低於其他三組(每五年YLD上升0.013 vs. 0.041、0.048、0.044)。結果顯示,性別、出生世代、居住地區與多重非傳染性共病型態等因素與YLD負擔軌跡分組顯著相關,其中居住地區之中南部、東部的「早期高負擔組」風險最高,OR分別為1.653與1.786,反映台灣長期以來因不同區域別產生的健康不平等問題。相對於「穩定低負擔組」,「低負擔上升組」、「中負擔上升組」與「早期高負擔組」之全死因死亡HR分別為1.376、1.884與3.523倍。除上述結果外,亦透過Upset plot觀察各軌跡組最常見的共病組合,發現「早期高負擔組」不僅同時罹患疾病類型最多,亦有一些其他軌跡組不會出現的組合,例如「心血管疾病+糖尿病與腎病」及「心血管疾病+神經系統疾病」等共病組合。
結論:
本研究利用縱向資料觀察YLD指標結合群組化軌跡分析,揭示台灣高齡人口健康負擔的異質性與長期發展途徑,結果顯示應以分群導向方式擬定健康老化政策之依據。未來建議將社會決定因素與跨部門健康促進策略納入考量,以改善弱勢群體於高負擔軌跡中所面臨的健康挑戰。
