論文名稱:基於機器學習探討阻塞性睡眠呼吸中止症患者使用連續性陽壓呼吸器之遵從性

背景:阻塞型睡眠呼吸中止症(OSA)是現今常見的睡眠呼吸障礙疾病,意思是說當一個人在睡眠中因呼吸道的阻塞,所導致間歇性呼吸停止,這樣的狀況必須符合每小時中有五次以上的呼吸暫停(apnea)或者是呼吸不足(hypopnea)的情形,每次10秒以上,並且伴隨著明顯的在大白天時候的精神不濟與疲倦感,配帶持續性楊壓呼吸器(CPAP)是目前治療的黃金標準。然而CPAP治療最主要的問題在於患者的遵從性,在剛使用CPAP治療的患者中,常常會不能適應CPAP所帶來的不適感而不願繼續使用。本研究主要是客觀透過回朔性來做檢查數據分析,從患者的基本資料與睡眠多項生理監測檢查(Polysomnography, PSG)以及壓力測定試驗(CPAP titration test)的記錄,亦考量患者電話詢問之內容,透過人工智慧,運用不同機器學習模型探討OSA病患使用CPAP的遵從性,來探討與找出影響病患使用CPAP的重要因素。

研究目的:本論文透過人工智慧,運用不同機器學習方式探討OSA病患選出前幾項最主要有可能影響患者攜帶呼吸器的特徵,透過這樣的方式來預測是否病患在未來對於攜帶陽壓呼吸器的遵從性可以更佳有意願。

研究方法:透過使用購買呼吸器且遵從性佳三個月以上以及未滿三個月之所有檢測的參數及受測者之年齡、性別、BMI作為特徵,透過機器學習訓練出的預測模型。

研究結果:本研究發現當把全部的檢驗測試的特徵投入做訓練的時候,透過卡方檢定選出來的特徵後,再以皮爾森積差相關分析找尋其相關聯性,最後運用機器學習的方式,其中可發透過決策樹演算法這些特徵對於攜帶呼吸器的遵從性相關聯之影響力之準確度以及靈敏度分別達到0.67與0.72。

結論:卡方檢定選出來的特徵,再以皮爾森積差相關分析學習法可以確認那些特徵指標的影響因子,來發現病患於對於攜帶呼吸器的遵從性是可有依據的,並透過深度學習模型是能夠辨識出來的。未來若是可以在呼吸睡眠檢測報告的輸入方式更新與轉換報告的錯誤更新之後,再請呼吸技師們再多確認實際資料的正確性,應該更能提高模型的準確與預判能力。這樣也有助於醫師或者睡眠技師,可以早日發現並提供連續性陽壓呼吸器給需要且會長長久久攜帶著的病患。
