Abstract
近年來,由於氧化鋯具備優異的美觀性、機械強度與生物相容性,已廣泛應用於牙科修復領域。其中植體材料逐漸從鈦金屬轉往氧化鋯發展,氧化鋯作為贋復物在經過表面處理改變表面物化性質能有效提升黏結強度,然而氧化鋯植體經過不同表面處理,對植入後的穩定度及術後恢復,包括生物相容性、骨組織貼附與生長及軟組織癒合等相關研究尚不明確。本研究旨在探討三種對氧化鋯的表面處理,分別是未處理(NT)、噴砂(AB)及玻璃陶瓷噴塗結合酸蝕(GE),並結合電漿處理(PL、ABPL、GEPL)對氧化鋯表面物理化學特性與生物反應之影響。
試片經不同表面處理後,使用場發射掃描式電子顯微鏡(FE-SEM)觀察表面形貌,搭配能量色散X射線譜(EDS)分析其元素組成,並測量表面粗糙度、接觸角與表面能評估改質效果。生物評估方面,透過人類骨母細胞(Human osteoblast-like cells)與人類牙齦成纖維細胞(Human Gingival Fibroblasts-1)進行細胞貼附、活性、毒性、發炎反應及骨代謝測試,以評估細胞行為與材料生物相容性。所有實驗數據均進行統計分析(p < 0.05)。
結果顯示,GE與GEPL組呈現棒狀與針狀玻璃陶瓷交錯的特殊結構,表面粗糙度與親水性顯著提升(p < 0.05)。電漿處理能有效降低表面有機雜質、提升表面自由能,並進一步改善材料潤濕性。細胞貼附實驗中,GEPL組表現出最佳之細胞貼附數量與形態展開程度;各處理組的細胞毒性與發炎因子(Tumor necrosis factor-⍺)表現皆無顯著上升,顯示各組處理皆具良好生物相容性。此外,GEPL組在鹼性磷酸酶(Human Alkaline Phosphatase)活性上亦顯著高於其他組別(p < 0.05),顯示具促進骨代謝潛力。
生物反應的測試可得知,經過不同表面處理的氧化鋯對於細胞的活性及生長不會產生抑制作用,且試片本身也不會釋放對細胞有害的毒性物質。細胞貼附的測試可觀察到,噴砂及玻璃陶瓷噴塗比起未處理組有較好的細胞貼附數量及細胞形態,再經過電漿處理有可以進一步的增加細胞貼附數量及表現出更好的細胞形態,其中玻璃陶瓷噴塗結合電漿處理有最好的表現。
綜合上述結果,玻璃陶瓷噴塗結合電漿處理(GEPL)對於氧化鋯的表面物化性質有顯著提升的效果(p < 0.05),可使氧化鋯具有親水及粗糙之表面,且不會釋放對細胞有害或誘導發炎之物質,同時具有最突出之細胞貼附、細胞活性及ALP活性表現,進而促進細胞在試片上有最好的生長表現。
