Abstract
本研究使用生物可降解高分子支架(Biodegradable polymer scaffold)做為載體,於支架上分別添加骨成形蛋白(Bone morphogenetic protein-2)、靈芝蛋白(Ling Zhi protein)及生理食鹽水(控制組),將上述材料分別填塞於兔子鼻竇骨上三個規格統一化的骨缺損。目的為測試生物可降解高分子支架及靈芝蛋白於活體骨組織之生物相容性,且觀察靈芝蛋白是否具有促進骨頭再生(Bone regeneration)之功效。
實驗使用十二隻雄性紐西蘭大白兔(年齡介於18至24週,體重介於3.3至3.8 kg)。於每隻大白兔之鼻骨位置使用內徑2.5 mm的Trephine bur進行鑽孔,小心地游離及移除覆蓋於上顎竇黏膜的圓形骨板,每隻兔子利用相同方式製造三個骨缺損,控制組於生物可降解高分子支架上添加生理食鹽水,實驗組則是使用骨成形蛋白及靈芝蛋白加上生物可降解高分子支架。液體材料與支架混合完成後,分別將三個組別的材料填塞入兔鼻骨上不同的骨缺損內。
實驗觀察週數為手術後一週、二週、四週及八週,每個觀察週數各犧牲三隻兔子。實驗結果分為立體導航式電腦斷層掃描分析(Volume computerized tomography)及組織學分析(Hematoxylin-eosin stain),探討骨癒合情況及材料的組織學反應。
電腦斷層掃描所得到的影像資訊分析可發現不同組別的材料在每個觀察時間點都有骨頭再生,骨成形蛋白在術後第四週之骨增生寬度及骨增生體積高於其他兩個組別,且差異有統計學上的意義,控制組於術後第八週之骨增生寬度較另外兩個材料組別低,且差異也具有統計學上的意義。
組織學檢查下,所有的樣本都沒有發現嚴重發炎(Hyper-inflammation)或異物反應(Foreign body reaction)。骨成型蛋白樣本之組織學檢查與斷層掃描結果一致,不論是骨再生或骨組織成熟度(Tissue maturation)在所有觀察週數都比另外兩組材料明顯。靈芝蛋白樣本於癒合初期觀察和控制組差異不大,但於癒合後期(第四週及第八週)可觀察到骨頭的緻密度和成熟度都高於控制組。
本研究結論為骨成形蛋白及靈芝蛋白於兔鼻竇骨皆有促進骨再生及組織分化的效果。組織學觀察下,所有樣本皆無嚴重發炎反應及異物反應,代表本實驗植入之材料(包含生物可降解高分子支架、骨成形蛋白、及靈芝蛋白)皆具有良好的生物相容性。
