Abstract
在全球數位醫療快速發展的趨勢下,建構標準化、結構化且具互通性的健康資訊交換機制已成為智慧醫療發展之關鍵。由HL7制定的「快速健康照護互通操作資源(Fast Healthcare Interoperability Resources, FHIR)」標準,具備模組化與高度擴充性,已被視為解決醫療資訊孤島的核心技術。衛生福利部中央健康保險署為解決癌症用藥事前審查流程中,因仰賴人工處理所導致的行政負擔與效率瓶頸,自2024年起推動「以FHIR標準申請癌症用藥事前審查」政策。惟現有文獻對於醫療機構在實際導入此一新興標準時,所面臨的實務挑戰與組織調適過程,仍缺乏系統性的探討。
鑑此,本研究旨在探討臺灣中大型醫院導入FHIR標準於癌症用藥事前審查之實務挑戰與影響因素。研究採用混合研究法,量性研究係運用台灣醫院協會於2025年2月進行之問卷調查資料,針對其中74家具申請經驗之醫學中心與區域醫院進行統計分析;質性研究則深度訪談中央健康保險署政策主管與醫院實務承辦人共七位,藉以整合量性數據與質性訪談結果,深入剖析制度脈絡。
研究結果顯示,資訊系統建置方式顯著影響導入準備度與挑戰類型。採自行開發模式的醫院,主要挑戰在於資料欄位轉換之複雜度;採委外開發模式的醫院,其挑戰則轉化為對廠商的溝通治理與合約限制。進一步分析顯示,曾參與中央健康保險署FHIR實作醫院者,其「FHIR導入現況與準備度」明顯優於未參與者,而投入FHIR的時間愈長,「技術能力與資源準備度」亦隨之提升,特別是在「IT團隊具備開發能力」等核心題項上,投入兩年以上之醫院呈現階梯式成長,反映試辦機制與長期投入可透過實作指引、測試驗證與錯誤回饋,形塑一套結構化學習路徑。
構面間相關分析亦顯示,「技術能力與資源準備度」與「對政策與未來發展的看法」之間呈現顯著正相關,且牽涉依FHIR實作指引開發、參與測試與技術驗證、具備資料轉換處理能力等題項,與醫院對「提升審查效率、強化資料治理並支援AI輔助審查」等政策效益的認同連動最為明顯,顯示唯有能將FHIR內化為具體臨床與作業賦能的醫院,較可能由「被動合規」轉為「主動認同」。
本研究結論指出,FHIR的導入不僅是技術規格的轉換,更是醫療流程再造的工程。但考量不同資訊體質的醫院在資源與技術能量上存在顯著落差,建議未來可採「分級化輔導」策略,針對委外醫院提供標準化轉換工具或定型化契約範本以降低門檻;同時應建立共享知識庫並深化技術賦能價值,協助醫院從單純的行政合規,邁向以數據驅動臨床價值之數位轉型。
