Abstract
人體關節軟骨的再生能力有限。利用組織工程與自體軟骨細胞移植的概念,將細胞與細胞支架 (scaffold) 結合,先於體外培養健康軟骨細胞組織,再經由注射方式修補受損關節軟骨,能提供一個有前景的治療軟骨方法。富含血小板纖維蛋白凝膠 (platelet enriched fibrin glue, PEF glue)可經由病患自體分離出的富含血小板血漿中加工製作而成。PEF glue屬於天然生物材料,結構完整能提供細胞貼附生長於其中,並具有獨特的生物相容與生物降解特性,能分泌多種促使軟骨分化生長的細胞激素提供細胞正常生長與分泌細胞外間質 (extra cellular matrix, ECM)。在本篇研究中,我們觀察到PEF glue含有大量的血小板於其孔洞中而且結構強度與完整性方面皆優於PRP gel 與bovine fibrin。另一方面,PEF glue 內所具有之TGF-β1 與 BMP-2細胞生長激素含量也明顯多於PRP gel與bovine fibrin。此外將不同細胞培養在三種材料中,經由細胞增生實驗得知在PEF glue 中,細胞生長增生狀況最好。而軟骨細胞培養在 PEF glue 中, type II collagen 跟aggrecan 的 mRNA 表現量與 PRP gel 相較都有上昇的趨勢,證實軟骨細胞在此材料中, re-differentiation 的效果最好。而在動物實驗組織切片方面,經由 Alcian blue 與 PAS 染色結果得知,培養於 PEF glue 中軟骨細胞產生之蛋白多醣 (proteoglycan) 與糖胺聚醣(glycosaminoglycan,GAG) 量優於在PRP gel 中。綜合以上實驗結果均可驗證PEF glue對於在促進軟骨再生方面的能力,明顯優於PRP gel與 bovine fibrin 。PEF glue為自體產生的材料含有豐富的生長因子,不具免疫問題,且注射方式傷口範圍以及感染問題皆縮小,也不需二次手術將細胞支架取出,在未來軟骨再生組織工程領域上具有明顯潛力。
