Abstract
腸躁症(Irritable Bowel Syndrome, IBS)不同於常見的腸胃疾病,並非腸胃出現發炎的病理狀況,也和細菌或病毒感染無關,而是腸道出現功能性問題。腸躁症與一些器質性的疾病於臨床表現及病生理機轉有重疊的現象。膽結石(Gallstones)是臨床上常見的消化道疾病問題,近年來隨著年齡老化與生活西化,膽結石發生率與重要性也持續上升。雖然大多數膽結石患者終其一生不會有任何症狀,但若有症狀出現,多半是右上腹疼痛、腹脹,結石卡在膽囊出口,會引起膽囊炎;掉到膽管,則會引起膽管炎。發炎時,除疼痛外尚會發燒、嘔吐、白血球升高,也會出現黃疸。結石若進一步掉到胰管開口處,會引發更嚴重的胰臟炎。目前尚無強而有力的證據表明膽囊疾病(Gallbladder disease)在腸躁症患者中更為常見,但此後可能與腸躁症發展或腸躁症症狀惡化有關,已有研究證實患有膽結石的受試者中,後續發生腸躁症的風險增加。

這兩種消化道問題都會造成病患生活上的不方便,對生活品質的影響不容小覷。此研究將進行回溯性研究以探討腸躁症與膽結石之間的相關性、臨床特點及相關因素於台灣人族群中是否存在。

研究方法:本研究為使用臺灣全民健保資料庫進行了一項10年期的全國性人口世代研究。2001 年至 2005 年篩選出 33,908 位腸躁症患者,依據性別、年齡及共病症以1:5的比例配對出 169,540 位對照組,研究腸躁症患者於10年內是否發生膽結石症,利用Cox 比例風險模式研究腸躁症與發生膽結石症之關聯性。

研究結果:於納入本研究的 203,448 位患者中在10年的追蹤期間,腸躁症組的 1,656 位(330313.46人年)被診斷有膽結石症,對照組有3,338 位(1681166.6 人年)被診斷有膽結石症。腸躁症患者發生膽結石症的風險高於無腸躁症者(校正風險比=2.528,95%信賴區間=2.384-2.682,P<0.001)。以性別而言,女性與男性發生膽結石症的風險均達到顯著差異,女性(校正風險比=2.344,95%信賴區間=2.141-2.566,P<0.001),男性(校正風險比=2.675,95%信賴區間=2.475-2.892, P<0.001)。年輕族群(20-39歲)患者發生膽結石症的風險較高,校正風險比= 3.229,95%信賴區間=2.749-3.793。根據不同期間(1年、3年、5年、8年和 10年)的亞組分析,腸躁症與膽結石症的關聯性依然顯著(均P<0.001),尤其以第一年的風險比為最高,校正風險比= 6.585,95%信賴區間=5.567-7.788,P<0.001。

結論:本研究結果顯示腸躁症患者比無腸躁症者有較高的罹患膽結石症風險,尤其是在追蹤的第一年期間。臨床醫生應該要留意這關聯性,在腸躁症患者要更為注意膽結石症發生的風險。
