Abstract
目的
利用Assessment of Spondyloarthritis International Society and the Outcomes Measures in Rheumatology collaboration (ASAS-OMERACT) 核心領域組合所提及的中軸性脊椎關節炎之重要指標相關且合適的工具,評估生物製劑和小分子藥物作為中軸性脊椎關節炎二線治療的益處與壞處。

研究設計
系統性文獻回顧與網絡統合分析。

資料來源
使用PubMed, Embase, Cochrane Central Register of Controlled Trials (CENTRAL), ClinicalTrials.gov, World Health Organization International Clinical Trials Registry Platform (WHO ICTRP)以及各大醫學會議的資料庫進行搜尋,設定的搜尋時間為從創刊至2024年2月29日。

挑選合格研究的標準
隨機對照試驗,其探討與成年中軸性脊椎關節炎患者相關的結果,受試者必須對非類固醇類消炎藥的使用無顯著治療效果,同時接受生物製劑和小分子藥物作為單一療法,以及直至該被納入之研究評估結果時的主要時間點。

結果
總共有38項研究(17種介入包括安慰劑和8808名受試者)被納入至網絡統合分析中。對於Axial Spondyloarthritis Disease Activity Score clinically important improvement (ASDAS CII),除brodalumab之外的所有藥物介入均顯著優於安慰劑(風險比範圍1.85至3.94,高至低證據可信度);只有ixekizumab顯著優於含負載劑量的secukinumab 150毫克(風險比1.66(95% 信賴區間1.04至2.65),中證據可信度),並排名第二最高(累積排行曲線下面積78.8%)。對於Axial Spondyloarthritis Disease Activity Score inactive disease (ASDAS ID),除filgotinib和tofacitinib 5毫克之外的所有藥物介入均顯著優於安慰劑(1.67至17.43,高至低);certolizumab pegol 200毫克顯著優於除adalimumab、certolizumab pegol 400毫克、靜脈輸注的golimumab、ixekizumab和filgotinib之外的所有藥物介入(4.14至10.41,高至低),並排名最高(93.7%)。對於Bath Ankylosing Spondylitis Disease Activity Index ≥50% improvement (BASDAI50),所有藥物介入均顯著優於安慰劑(1.88至4.90,高至低);infliximab顯著優於除adalimumab、certolizumab pegol 200毫克和400毫克、靜脈輸注的golimumab和ixekizumab之外的所有藥物介入(2.23至2.63,中至低),並排名最高(95.6%)。對於Assessment of SpondyloArthritis international Society 40 improvement criteria (ASAS40),除filgotinib之外的所有藥物介入均顯著優於安慰劑(1.59至8.83,高至低);netakimab顯著優於除靜脈輸注的golimumab和infliximab之外的所有藥物介入(2.70至5.56,低),並排名最高(98.1%)。對於adverse events,ixekizumab(1.18(1.03至1.36),高)和含負載劑量的secukinumab 150毫克(1.14(1.05至1.25),高)相較於安慰劑會帶來更多的壞處;藥物介入之間未觀察到差異具有顯著差異(高至低)以及安慰劑排名最高(80.6%)。對於withdrawals due to adverse events,所有比較組合的差異均無達統計學上的意義(中至非常低)以及含負載劑量的secukinumab 150毫克排名最高(68.1%)。對於infections,ixekizumab相較於安慰劑(1.55(1.07至2.26),中)和upadacitinib(1.61(1.01至2.58),低)會帶來更多的壞處以及upadacitinib排名最高(71.6%)。當著眼於所有主要結果上皆有數據的藥物介入,並且透過效果估計值與其信賴區間和排名順序進行比較時,發現adalimumab和ixekizumab在有效性方面的效果較大,但ixekizumab可能帶來更多壞處,adalimumab則在安全性方面的排名較差;upadacitinib在安全性方面雖然有好處,但其有效性具有局限性;etanercept 50毫克、含負載劑量的secukinumab 150毫克150毫克和tofacitinib 5毫克至少在有效性和/或安全性方面至少有一個不一致的發現。

結論
整體而言,adalimumab、ixekizumab和upadacitinib具有完整且有利的概述,但adalimumab和ixekizumab可能引起安全性問題,而upadacitinib在有效性方面可能提供有限的效果。雖然etanercept 50毫克、含負載劑量的secukinumab 150毫克和tofacitinib 5毫克的概述是全面的,但在有效性和/或安全性的發現具有不一致性且無法作出強而有力的歸納與總結。儘管許多藥物介入(包括certolizumab pegol 200毫克和400毫克、靜脈輸注的golimumab、infliximab以及netakimab)在至少其中一個的有效性指標有顯著的益處,但其部分的有效性和安全性資訊尚無法取得。鑒於目前缺乏具有直接比較證據的文獻,我們認為此篇研究的結果可為利害關係人提供較為完善的醫病共享決策之信息,並運用於實證臨床照護中。
