Abstract
疼痛為加護病房最常面臨的問題,當疼痛一直無法獲得緩解,可能會增加患者罹病率及致死率,病人自我報告被認為是疼痛評估的最佳方式,但並不適用於所有族群,目前臨床上的疼痛評估工具太多,針對困難評估病人,特別是意識不清患者,在疼痛評估上並無一黃金標準,因此本研究藉由意識清楚且使用呼吸器患者,以可自我報告之視覺類比量表(Visual Analogue Scale,VAS)為基礎,來比較疼痛行為量表(Behavior Pain Scale, BPS),重症照護疼痛觀察工具 ( Critical Care Pain Observation Tool, CPOT),以及本研究機構目前使用之成人困難評估量表之信、效度。
本研究採探測性研究設計,以立意取樣法收集資料,在北部三間醫院的內外科及呼吸科加護病房進行收案,收案對象包括:(1)年齡20-89歲;(2)意識清楚,在插管下格拉斯哥昏迷指數(GCS)=11;(3) 經說
明研究目的後願意參與研究者;排除條件包括:(1) 使用鎮靜劑患者;(2) 使用神經肌肉阻斷劑患者;(3)呼吸器使用不足1天或超過30天;(4)加護病房住院天數不足1天。研究工具選用VAS、BPS、CPOT,以及成人困難評估量表,分別於三階段做測試:(1)休息時(2)給予病人抽痰刺激後立即評估 (3)抽痰後20分鐘(恢復期)。
本研究共收集77為符合條件之個案,結果顯示BPS、CPOT,以及成人困難評估量表內在一致性佳,刺激下Cronbach's α 分為 .80、.73、.61;BPS量表在不同狀態下的評分一致性較穩定且信度高,皆維持在80%,CPOT及成人困難評估量表評分一致性介於60-80%。效度方面顯示三種疼痛困難量表效標關聯效度佳且差異性不大,皆可區辨在不同情境下的疼痛差異及強度(p<0.0001),ROC Curve計算BPS、CPOT、成人困難評估量表最佳切點,依序為5分、3分、3分,但藉由敏感度及特異性計算曲線下的面積(Area Under Curve, AUC),依序為成人困難評估量表(.83)、BPS (.77)、CPOT (.70)。此外本研究發現刺激下,三種疼痛困難量表所測得之疼痛程度皆低於自我報告(VAS)。刺激狀態下疼痛的影響因子,教育程度較低者與內科病人疼痛分數會較高(p < 0.05)。
由於BPS量表在信效度各方面皆十分良好且相對穩定,本研究建議BPS量表為三種困難評估工具中較好的困難評估工具,期許本研究成果能夠改善護理人員針對困難溝通族群的疼痛評估,以作為臨床疼痛處置之有效的依據。
