Background: In Paraguay, one common problem that public hospitals face is ineffective administration, which prevents all Paraguayans from receiving quality health care. As a result of this issue, the Ministry of Health and Social Welfare and the Republic of China-Taiwan government decided to launch a project called "Improving Efficiency in Health Information Management." It encourages the use of open-source software to create a health information framework and improve health information management efficiency through the development, installation, use, and promotion of associated applications, as well as the development of health administration capacities.
Objective: This study aims to analyze whether structure, process, and outcome can be used to evaluate the quality of the health information system and investigate the health workers’ perceptions and the relative effectors of the quality of the new system implemented in three public hospitals in Paraguay.
Methods: Based on the Donabedian model, a questionnaire was developed and validated by a group of experts in the development and implementation of the HIS system in Paraguay. We recorded and analyzed the data in the SPSS PC Statistical Package version 18. The descriptive statistics included the frequency and percentage.
Results: Out of 439 respondents, 54.7% were female, from 30 to 39 years old (44.9%), coming from the General Hospital of Limpio (53.28%), working as nurses (51.5%). Pearson’s correlation showed significant associations between structure and process (p<0.001), structurer and outcome (p<0.001), and process with outcome (p<0.001).
 
Conclusion: our findings indicated that most users agree that the department/unit has adequate infrastructure, with sufficient resources and competence to implement the system, these qualities being part of the structure. There was not much discrepancy in the responses obtained for the domain of results, where most users also agreed that the system is easy to understand, improving their job performance, as well as, improving the patient care delivery, making the HIS system useful for its purpose.
By evaluating the open questions, HIS users are demanding more training, as well as more personnel to help when they find a problem with the system (corresponding to the domain of process).
