Abstract
視網膜退化性疾病,如老年性黃斑部病變 (Age-related macular degeneration, AMD) 的盛行率隨著全球平均壽命的提高也顯著上升。許多研究指出神經發炎在AMD會造成視網膜損傷加劇,導致血視網膜屏障破壞與周邊單核球及巨噬細胞浸潤,也伴隨視網膜膠質細胞的過度活化。治療眼科疾病的傳統中藥菟絲子 (Cuscuta chinesis) 中含有天然黃酮類Isorhamnetin,其具有多種生物活性。本研究旨在使用細胞實驗和NaIO3誘導之小鼠AMD模型探討Isorhamnetin對於視網膜之保護作用與機制。在細胞實驗中,Isorhamnetin顯著抑制了單核球細胞 THP-1在TNF-α、IL-1β或脂多糖LPS刺激下基質金屬蛋白酶MMP-9的活性與蛋白質表現量;同時,也顯著降低了THP-1細胞中TNF-α刺激下MMP-9 mRNA轉錄。小膠質細胞BV-2實驗中,Isorhamnetin能顯著降低LPS刺激下一氧化氮NO的產生和釋放。細胞訊號路徑的實驗結果顯示,Isorhamnetin主要抑制了THP-1細胞經TNF-α刺激後上調的Src和AKT磷酸化。視網膜色素上皮細胞ARPE-19實驗中,Isorhamnetin無法保護ARPE-19在NaIO3或tBHP誘導氧化刺激下的存活率,但可顯著抑制在TNF-α刺激下上調的MMP-9的活性,顯示Isorhamnetin作用偏向抗發炎而非透過抗氧化作用。動物實驗方面,在NaIO3誘導之小鼠AMD模型中,視網膜功能性分析發現Isorhamnetin能顯著保護視網膜電圖 (Electroretinography, ERG) 的a、b、c波振幅 (Amplitude) 與b波潛期 (Latency),眼科光學斷層掃描OCT顯示Isorhamnetin能維持視網膜厚度與結構,色彩瞳孔光感反射 (Chromatic pupillary light reflex, cPLR) 分析中,Isorhamnetin亦能顯著改善小鼠之光感反射功能。此外,Isorhamnetin 能保留免疫螢光染色 Opsin表現、維持RPE結構蛋白ZO-1的完整性、減緩GFAP和MMP-9過度活化表現並降低CD11b⁺免疫細胞的浸潤數量,更抑制視網膜中上調的MCP-1與MMP-9活性,顯示其具調控神經發炎的能力。綜合本研究之細胞與動物實驗發現,傳統中藥之活性天然物Isorhamnetin透過抑制神經發炎及免疫細胞活化並展現對視網膜功能與結構的保護效果。
