Abstract
多型性神經膠質母細胞瘤(以下簡稱原發腦癌)是最常見的原發性惡性腦瘤,由於腫瘤本身源自於腦實質組織,且腫瘤本身以浸潤性的生長方式擴散,幾乎不可能採取廓清式的手術切除方式治療,儘管手術後配合標準的放射線與化學治療,但整體預後與病患平均存活時間始終維持在15個月左右。丙戊酸(Valproic acid, VPA)是臨床上常使用的抗癲癇與抗精神病用藥,具有抑制histone deacetylase (HDAC)的活性。臨床研究分析也指出,使用丙戊酸的原發腦癌病患確實有較長的存活期,因此我們試著探討丙戊酸抗原發腦癌的分子機轉。給予原發腦癌細胞株丙戊酸5至20 mM時,可抑制細胞的生長,引起細胞週期停留在G2/M期,並產生過多的reactive oxygen species (ROS),進一步分析發現丙戊酸增加p27、p21與Bim蛋白質的表現,同時抑制了paraoxonase 2 (PON2)、cyclin B1、cdc2與Bcl-xL蛋白質的表現。此外,分析原發腦癌病理標本,發現癌組織確實表現較高量的PON2蛋白質,這個結果與TCGA資料庫的分析結果是一致的。以基因轉殖方式在原發腦癌細胞株中大量表現PON2,發現丙戊酸的處理可大量降低細胞內的ROS及Bim的表現;反之,以基因壓抑技術(gene knockdown technology)方式壓抑PON2的表現,發現丙戊酸的處理可大量增加細胞內的ROS及Bim的表現。進一步在原發腦癌細胞株中轉殖PON2啟動區報導基因(promoter reporter gene),發現丙戊酸調控PON2基因表現的位置位於PON2基因上游-400~-1 bp。在裸鼠異體移植原發腦癌細胞的活體試驗中,我們發現丙戊酸可以有效抑制腫瘤細胞的生長,且可降低腫瘤組織表現PON2,而增加Bim的表現。綜合我們的研究,顯示丙戊酸可經由抑制原發腦癌細胞PON2基因的表現,導致ROS大量生成及Bim的表現,最後引起腫瘤細胞的凋亡。
