Abstract
背景與目的: 根據國健署統計, 2021 年台灣每日約新增 44 例大腸癌病例,且 40 至 49 歲族群的發病率較上一代增加 1.5 倍,引發對年輕族群大腸癌風險的關注。專家建議將公費篩檢年齡自 50 歲下修至 45 歲,預估十年可減少 1,800 例癌症。隨健康意識提升,腸胃內視鏡成為大腸癌早期篩檢的重要工具。為了減輕檢查不適,多數患者會接受鎮靜麻醉,其中 Propofol 為常用藥物,但劑量需仰賴臨床醫療人員經驗決定,難以兼顧個別差異,恐有過量風險,影響檢查安全與品質。本研究在建構Propofol 需求劑量預測模型,協助臨床於無痛腸胃內視鏡中調整鎮靜策略,提升安全性與精準性。
方法: 研究資料以單一中心健檢麻醉紀錄為基礎,納入受檢者之年齡、性別、 BMI 、病史、檢查時間及用藥等資訊,嘗試運用多種監督式機器學習方法建構預測模型。模型效能以 AUC 與準確率為主要評估指標,並參考 Precision 與 Recall 等數據進行比較與優化。預測目標為辨識 Propofol 劑量是否高於建議值(>1.2 mg/kg),以提前發現高劑量潛在使用者,輔助臨床術前劑量判斷。
結果:結果顯示, 所建構的機器學習模型在預測 Propofol 藥物需求劑量表現良好 ,其中以 XGBoost 模型的 AUC(0.699)與準確率(0.646)最為穩定,整體評估指標亦優於其他模型。該模型能有效辨識建議劑量與高劑量使用個案,未來若進一步整合於臨床醫療系統,有潛力作為術前風險辨識工具,協助臨床醫療人員針對潛在高劑量使用者調整鎮靜策略,以提升檢查安全性與減少藥物不良反應風險。
結論:此研究成功整合健檢腸胃內視鏡的臨床資料與機器學習,成功建構 Propofol需求劑量預測模型,不僅提升分類效能,也回應個別化鎮靜需求。模型輔助術前判斷高劑量使用者,有助提升用藥安全與檢查品質。SHAP 解釋機制亦強化模型透明度,具臨床應用潛力,未來可拓展至其他臨床麻醉決策支援。
