Abstract
前言:
中耳炎好發於學齡前兒童,其發生率隨著年齡增長而明顯下降。在已發表的相關研究中,發現一些空氣污染物、氣候因素與中耳炎發生可能有關,但研究結果仍然存在很大的不一致性。為此本研究以大樣本的人口學為基礎,目的在釐清臺灣空氣污染物和氣候因素與0-18歲族群中耳炎發生的相關性。

研究方法:
本研究選取全民健康保險研究資料庫LHID2005年抽樣檔中2010年1月1日至2013年12月31日主診斷或次診斷碼為381.00 - 381.06、381.4、382.00 - 382.02、382.4以及382.9之急性中耳炎門診紀錄且就診時年齡≦18歲之個案作為研究對象;空氣污染物濃度以及氣候因素資料則來自臺灣環境保護署環境資源資料庫,選取各空氣品質監測站之污染物日值,利用斯皮爾曼等級相關係數以及ARIMA回歸模型評估空氣污染物和氣候與感染中耳炎的相關性。

研究結果:
0-18歲的研究族群中,只有延遲1-3天的相對濕度及延遲3天的降雨量與中耳炎發生率之斯皮爾曼相關係數未達統計學上的顯著。調整季節及趨勢後,延遲天數為0及1天的ARIMA時間序列分析模型中0-18歲族群之急性中耳炎發生率與PM2.5及氣溫相關性達統計學上的顯著;延遲天數為2天的模型中0-18歲族群之急性中耳炎發生率只與氣溫相關性達統計學上的顯著;延遲天數為3天的模型中0-18歲族群之急性中耳炎發生率與PM2.5、氣溫及相對濕度相關性達統計學上的顯著。

研究結論:
研究結果發現,0-18歲族群急性中耳炎發生與部分空氣污染物及氣候因素有關。空氣污染物中PM2.5濃度與延遲0、1天以及3天的急性中耳炎發生率之間的正相關性最為強烈,臭氧則沒有發現與急性中耳炎發生率有關;氣候因素中氣溫與延遲0-3天的急性中耳炎發生率之負相關性最為明顯,相對濕度則與延遲3天的急性中耳炎發生率有正相關性,沒有證據顯示降雨量與急性中耳炎發生率有關。
