Abstract
良好的術後照護可減少合併症發生,減少醫療照護費用支出,進而提升國民健康生活品質。本研究以頭頸癌病患接受游離皮瓣重建手術之後的照護結果為例,探討與良好術後照護結果相關之指標因子。結構式問卷調查及病歷回溯方式收集資料,採縱貫式研究法(Longitudical study),研究工具包含華盛頓大學生活品質量表第四版(the University of Washington Quality of Life, UW-QOL v4)中文版和病歷回溯方式,定期追蹤收集資料,直到病患死亡或觀察期截止。Cox比例危險模式(Cox proportional-hazard Regression model)進行檢定預測腫瘤局部區域控制、無遠端轉移存活時間和整體存活時間,結果顯示在生活品質單一指標中,手術前整體生活品質、身體功能相關生活品質及社交情感功能相關生活品質等因子,可以預測無遠端轉移存活時間與整體存活時間,尤其是身體功能相關生活品質更具有顯著的指標。可以預測照護結果的指標因子如,外觀、吞嚥、咀嚼、講話與口水分泌等,在腫瘤局部區域控制及整體存活時間都有良好預測能力。所以利用生活品質當成預測指標,尤其是身體功能相關生活品質,可以有效預測無遠端轉移存活時間和整體存活時間,以期許規劃完整治療方針與照護目標,讓病患及家屬能積極接受治療,持續定期追蹤,早期發現早期治療,提升整體照護結果。
