Abstract
背景:現今全球有超過4700萬的病患失智症病患,預計到2050年將增加至1億3150萬人。失智症是一群症狀的組合,包括認知能力逐漸惡化、同時可能出現干擾行為。因此,本研究探討失智症住院患者是否因症狀及干擾行為,而導致事故傷害與死亡風險增加,以及藥物治療對於症狀與干擾行為的成效,以利未來提供適當介入措施。
研究方法:本研究分三階段進行,第一階段為比較有無失智症患者整體事故傷害及各項危險因子,資料來源為2000年至2013年全民健康保險研究資料庫,診斷為失智症之住院病患。非失智症患者依照性別、年齡分層進行1:4配對;使用Cox 迴歸模式分析事故傷害危險因子。第二階段分析失智症在精神科住院的各項特性及住院死亡影響因子,資料來源為1997至2010年的全民健康保險研究資料庫中承保抽樣歸人檔(2005)。並根據DSM-IV TR診斷標準,第一軸ICD-9- CM診斷第一個代碼為290;排除有嚴重精神與酒藥癮病史者,使用Cox 迴歸模式分析死亡風險與危險因子。第三階為使用統合分析探討單一藥物治療及組合療法的使用策略對治療失智症中重度患者認知功能、精神行為症狀、自我照顧能力及整體功能成效與其安全性評估。
研究結果:第一階段結果為因事故傷害而住院之危險因子為失智症(尤其是阿茲海默症)、女性、年齡65-74歲、一年前曾經因事故傷害至門診或急診就醫。失智症者因整體事故傷害而住院的比非失智症者高。第二階段結果顯示在精神科住院死亡的危險因子為男性、高齡、伴隨瞻妄與伴隨妄想症狀。第三階段統合分析共納入11篇文章,組合性藥物治療(Donepezil合併Memantine)比單一使用治療(Donepezil)對於認知功能為中度效果量、行為精神症狀及整體功能評估皆為高度效果量;組合性藥物治療與單一使用治療相比,並不會增加藥物的不良反應。
結論:對於失智症患者給予預防事故傷害的護理指導。另外,針對失智症在精神科病房住院病患及其家屬,除了給予行為精神症狀治療與相關衛教外,還需預防呼吸系統感染,降低住院死亡比例。中重度失智症治療,建議組合性藥物治療成效優於單一藥物治療。
