Abstract
在輸血治療中,血袋內之白血球及細胞所衍生之生物激素被認為是導致病患產生輸血不良反應的重要原因之一,因此在許多歐美先進國家,白血球之全面移除 ( 以下簡稱減白 ) 已經是國家之重要用血政策。減白之時機點分為儲存前及儲存後移除,一般認為儲存前減白血品可有效移除白血球所衍生之生物或細胞激素,並有效降低因為生物或細胞激素所引起之輸血反應,台灣因為健保制度之關係,目前無法全面做到儲存前之減白,因此在考量使用之方便性及時效性,醫療院所也會在院內進行儲存後之減白。為瞭解儲存前、後減白血品所導致輸血不良反應是否有所差異,我們回溯2009年7月起至2013年12月止,統計儲存前減白紅血球 (以下簡稱Pre-LPR) 、儲存後減白紅血球 (以下簡稱Post-LPR) 之間,以及儲存前減白分離術血小板 (以下簡稱Pre-LPH) 、儲存後減白分離術血小板 (以下簡稱Post-LPH) 之間所引起之各類型輸血反應發生率;其中,經羅吉斯迴歸分析後,僅Pre-LPR引起之非溶血發燒輸血反應發生率為0.15%,Post-LPR為0.55%,具有統計學上顯著差異 (p<0.05) 。此外,Pre-LPH及Post-LPH引起之各類輸血反應發生率皆無顯著差異。
為瞭解影響輸血反應率差異之原因,我們針對不同減白製程血品在儲存過程中所累積之生物或細胞激素進行檢測,以瞭解其與輸血反應發生率是否有其關連性。過去文章多以酵素免疫分析法進行細胞激素之測量,而本研究則以螢光微(磁)珠Luminex Bioplex system進行量測,具備較高之敏感度及較低之檢測極限,以探討儲存前及儲存後減白血品在經過保存之後,其細胞激素之產生是否有統計學上顯著之差異。我們在Pre-LPR及Post-LPR經過保存之第4天及第42天,Pre-LPH及Post-LPH則在經過保存之第2天及第5天,量測12種細胞激素,包括IL-1α, IL-1β、IL-2、IL-4、IL-6、IL-8、IL-10、IFN-γ、MIP-1α、RANTES、TNF-α、VEGF;其中,在減白紅血球之類別中,儲存前減白製程可有效降低IL-1β, IL-8, RANTES及VEGF在儲存過程中之累積,而在在減白分離術血小板之類別中僅可有效降低IL-1β之堆積。
