Abstract
研究背景
先天性聽力損失為新生兒常見的感覺障礙疾病,其對語言發育有負面影響並可能影響未來就業。接受早期人工耳蝸植入的聽力受損兒童在言語發展、溝通、社交和學習成績方面皆有正面效益。健康效用值是代表受試者對特定健康狀態的偏好程度。其值可用於成本效用分析研究,作為臨床決策及衛生政策計畫的參考指標。本研究旨在測量雙側聽損使用不同聽覺輔具的健康效用值。
研究方法
這是一項橫斷面研究,使用視覺類比尺度量表和時間交換法測量四種不同健康狀態的健康效用值。受訪者包含聽損病人組和醫療人員組。在病人組中,效用值是根據病患目前健康狀態測量而得,而於醫療人員組則使用假想情境。測量的健康狀態包括未使用輔具、雙側助聽器、雙模式輔具(單側人工耳蝸植入者之對側耳持續使用助聽器)和雙側人工耳蝸。單因子變異數分析及事後檢定用於探討不同健康狀態之間效用值的差異。線性迴歸分析用於分析受訪者與健康效用值之間之關聯性。
研究結果
共37 名醫療專業人員及 134 名病人(55 名雙側助聽器、47 名雙模式輔具和 32 名雙側人工耳蝸)完成問卷訪談並納入分析。醫療人員組使用視覺類比尺度量表測得之平均健康效用值為 0.31(未使用輔具);0.41(雙側助聽器);0.63(雙模式輔具);0.82(雙側人工耳蝸)。使用時間交換法的平均健康效用值則分別為 0.60、0.69、0.81 和 0.90。其四種健康狀態的平均效用值統計上有顯著差異 (p<0.001)。在病人組中,雙側助聽器的視覺類比尺度量表測量值為 0.66; 雙模式輔具為 0.74;雙側人工耳蝸為 0.76。時間交換法測量值則分別為 0.75、0.84 和 0.86。三組平均值的差異具有統計學上顯著差異(p=0.016; p=0.002)。在線性迴歸分析結果中,較佳裸耳聽力具有顯著較高的視覺類比尺度量表測量值(全部三組:p < 0.001;雙模式輔具和雙側人工耳蝸組: p = 0.013)。
結論
研究結果顯示,使用視覺類比尺度量表及時間交換法測得之效用值,雙側助聽器最低;雙模式次之;雙側人工耳蝸最高。此外,患者組的線性迴歸分析結果表明,較好的裸耳聽力可得到較高之視覺類比尺度量表測量值。這是台灣第一個也是少數研究使用時間交換法來測量不同聽覺輔具於雙側聽損的健康效用值。此研究測得之健康效用值可作為未來成本效用分析研究的重要數據。本研究結果亦有助於臨床醫療人員闡明不同輔具改善聽損病人的生活品質及聽力之臨床效益
