Abstract
登⾰病毒(Dengue virus, DENV)是全球重要的⼀種由伊蚊傳播的⿈病毒,造成反覆的流⾏並導致顯著的發病率與死亡率。預測顯⽰隨著都市化與氣候變遷,受威脅的⼈⼝將持續擴⼤;然⽽,由於保護性與致病性 T 細胞反應之間複雜的相互作⽤,⽬前仍缺乏具備臨床參考價值的 T 細胞受體特徵(TCR signature)以辨識重症相關的免疫反應 。 由於 T 細胞的抗原特異性是由 T 細胞受體(TCR)的互補決定位區 3(CDR3)所定義,本研究對 225 名患者的隊列進⾏⾼通量TCRβ 定序,以探討特定 TCR 特徵是否與疾病嚴重度相關。在受體庫層級(Repertoire level),嚴重組呈現顯著的多樣性指數偏移與多個 TRBV 基因⽚段的使⽤偏好,反映出受體庫恆定模式(Homeostatic pattern)的重塑。為了識別潛在的抗原驅動訊號,我們應⽤了基於 CDR3 的序列趨同分析(CDR3 convergence clustering),將數千個獨特序列濃縮為 16 個⾼信⼼的嚴重度富集分群(Severity-enriched grouped clusters)。這些嚴重度富集 TCR 標記的總豐度與 DENV 相關系統性發炎的臨床指標呈現顯著相關,特別是⽩⾎球計數峰值與⾎⼩板波動。綜合⽽⾔,本研究證實了趨同的 TCR 模式可作為辨識 DENV 免疫病理狀態的分⼦指標,不僅揭⽰了重症患者特異性的 TCR 受體庫特徵,亦為未來開發精準的免疫監測⼯具與評估 T 細胞疫苗效⼒提供開發基礎。
