Abstract
隨著醫療衛生政策與民眾就醫習慣的改變,民眾對於社區藥局處置輕微病症及自我用藥的諮詢需求日益上升。為強化藥學系學生在社區藥局實務中的臨床諮詢能力,本研究旨在探討於輕微病症與自我照護課程中,導入案例導向教學結合生成式人工智慧(AI)工具之創新教學成效。
本研究採連續兩梯次之教學設計,研究期間透過問卷調查、期末學習考核、OSCE臨床技能模擬演練,以及生成式 AI 系統後台之互動紀錄進行數據蒐集與量化分析。評估指標涵蓋學生之生成式AI學習信念、病人照護自我效能之前後測變化、期末考試與 OSCE 測驗成績及課程滿意度,並進一步收集學生演練生成式 AI 模擬案例之對答文字紀錄與系統回饋,用以比較生成式 AI 與專家評估回饋之間的差異,藉此多維度評估教學成效。
本研究兩學期共納入 23 位學生。前後測比較顯示,學生對 AI 導入課程的正向學習信念與病人照護自我效能均顯著提升,而對 AI 的負面學習信念方面的擔憂則無明顯改變;質性分析亦顯示,學生提及「AI 能提供反覆練習機會」的描述比例有明顯增長。在滿意度方面,各面向之平均得分均達 4.5 分以上,顯示學生對此系統具高度易用性與使用意願。OSCE 考核中,學生在所有項目之得分均達 2 分以上(即達到部分期待),其中諮詢流程相關表現最為良好,但「告知病人用藥後症狀變化」則是學生最易遺漏之項目。此外,學生在生成式 AI 演練之分數,與藥師專家對 AI 文本之共識評分呈現顯著正相關;然而,在逐項比對中,亦發現 AI 評分存在評估標準前後不一及誤判關鍵字之偏誤。
本研究顯示,將生成式 AI 模擬諮詢系統導入藥學臨床實務課程,能有效提升學生對於臨床諮詢之自我效能,且學生對此創新教學科技普遍展現出良好的接受度。生成式 AI在自動化評量具備一定程度的可靠性,可作為輔助之能力指標,但因可能存在未被發現的偏差因素,仍需藥學專家進行定期回顧與動態修正,方能確保評估的準確性。
