Abstract
研究目的:近年來,新型熱塑性聚合物材料聚醚酮酮(polyetherketoneketone;PEKK)引入牙科領域,是個新的研究課題。本研究通過20年使用年限的臨床模擬戴入與移除測試來評估PEKK熱塑性樹脂牙鈎的固位力,試圖尋求理想的活動局部義齒(Removable Partial Denture;RPD)新型材料。

研究方法:本研究以市售上顎第一象限後牙區域的部分牙弓為主模型,將固位臂設計兩種不同尺寸(分別組別1-寬度:1.1-2.5mm厚度:1.1-1.3mm;組別2-寬度:1.5-3.0mm厚度:1.5mm)及三種不同倒凹深度(分別是0.25mm和0.5mm和0.75mm)之RPD的亞克斯牙鈎(Akers clasp)。通過電腦輔助設計和製造(Computer aided design/manufacturing;CAD/CAM)銑削由PEKK材料製作而成的RPD牙鈎(每組五個,共有30個研究試片),並藉由疲勞性質與固位力試驗評估牙鈎的固位性。使用兩因子變異數分析(Two-way ANOVA)與Tukey’s多重比較檢定(Tukey’s multiple comparison test)進行分析比較(p <0.05)。

結果:首先,本研究將不同尺寸的固位臂進行固位力比較,發現在組別2初始循環的固位力數值(2.80-3.51 N)表現出比組別1(2.40-3.36 N)更好的固位性。由此可得,固位臂尺寸的設計對牙鈎固位力有顯著影響。接下來,本研究將組別2的固位臂設計成不同倒凹量,進行固位力比較。由實驗數據可得,進入更深的倒凹量(0.75mm)的固位力(N)相較0.25mm與0.5mm高。此試驗印證,過去的文獻結果,以更深倒凹量設計的熱塑性樹脂牙鈎,較能獲的臨床可接受的固位力;除此之外,所有組別的初始循環固位力數值相較起來有顯著差異(p <0.043)。

結論:據研究顯示,高分子材料經過30,000次的牙鈎戴入與移除試驗後,仍保有一定的固位力;且透過不同倒凹量的模型實驗數據可以證實,牙鈎的倒凹深度經適當設計,能有效提升固位力。由於新型PEKK材料的實務應用對於牙科領域相對陌生,期許本研究針對PEKK材料應用在RPD牙鈎的相關機械性質探討,能夠深入了解創新RPD材料的特性,以供作牙科臨床應用參考。
