Abstract
癌症的轉移和進展是腫瘤發病機制中的重要關鍵,特別是在肺癌疾病過程中,它顯著導致器官功能障礙和生存率下降。儘管肺癌的進展和轉移的具體分子機制尚未完全明了,但最新研究顯示,腫瘤微環境中的系統性炎症反應在癌症發展中扮演著關鍵作用。這種炎症反應亦成為判斷癌症患者預後的一個重要標記。近期研究揭示,腫瘤微環境中的炎症反應在肺癌的發展過程中具有決定性的影響。本研究的目的是探究發炎小體(inflammasome)在肺腺癌(Lung adenocarcinoma,簡稱LUAD)中的作用,及其對肺癌轉移和進展的影響,以期為肺癌的治療提供新的思路和策略。
炎症體作為先天免疫反應的一部分,通過釋放細胞因子影響腫瘤細胞的生長、血管形成、免疫抑制、侵襲性及轉移。這些功能的活化與多種癌症的發展密切相關。本研究特別聚焦於Absent in melanoma 2 (AIM2)發炎小體,在LUAD中的表現指數及其與疾病進展的相關性,探討其作為癌症治療目標的可能性。發炎小體的激活釋放的細胞因子能引發腫瘤內的慢性炎症反應和免疫抑制,這在癌症轉移過程中扮演了關鍵角色。
我們使用來自癌症基因組圖譜(The Cancer Genome Atlas,簡稱TCGA)的LUAD數據庫進行初步的數據分析,比較原發性肺腫瘤中AIM2的mRNA指數與正常組織之間的差異。接著,分析AIM2 mRNA指數與肺腺癌患者的臨床特徵(如吸菸史、EGFR/KRAS/ALK基因突變狀態)及預後之間的相關性。為了評估AIM2炎症體在肺腺癌轉移中的功能,我們進行了基因沉默和過度表現實驗,這有助於識別AIM2炎症體在促進或抑制肺腺癌轉移中的具體機制。此外,我們探索了AIM2炎症體活性增加與免疫相關基因表現,特別是PD-L1的相關性,並評估了AIM2炎症體對腫瘤微環境中免疫反應的調節作用。
TCGA數據庫分析顯示,AIM2在原發性LUAD腫瘤中的表達顯著高於正常組織。高表現的AIM2與不良預後相關,尤其與吸煙史及EGFR/KRAS/ALK基因突變有關。功能性研究顯示,AIM2的活化直接增強了LUAD細胞的轉移能力,而其沉默則降低了細胞的遷移與集落形成能力。此外,AIM2指數的增加與免疫檢查點基因表現的上調相關,尤其是PD-L1,這是通過改變LUAD細胞中NF-κB/STAT1活性所介導的。這些發現強調了AIM2在肺腺癌的轉移和免疫逃逸中的關鍵作用,表明其作為治療干預的潛在目標。
