Abstract
Nε-carboxymethyl-Lysine (CML) 是高度醣化終產物 (advanced glycation end-products, AGEs) 中最常見的一種,在糖尿病併發症中扮演重要角色。活性氧化物質 (Reactive oxygen species, ROS) 的生成與粒線體的功能異常會誘導上皮─間質轉換 (Epithelial-mesenchymal transition, EMT),進而造成糖尿病腎臟纖維化。本篇論文以CML (6 μM) 和硫辛酸 (α-Lipoic acid, ALA) 作用於人類腎小管上皮細胞 (HK-2 cell) 中,並觀察其對細胞的影響。給予CML會導致ROS增加、粒線體膜電位降低並造成粒線體分裂,以硫辛酸進行前處理可以改善此現象。此外,給予CML會使HIF-1α和TWIST蛋白表現量會上升,進而誘導EMT的發生,使上皮細胞轉變為間質細胞,且α-SMA螢光表現量增加,以ALA進行前處理可以降低CML誘導的HIF-1α和TWIST蛋白表現量、抑制EMT、使細胞恢復成上皮細胞型態。綜合上述內容,我們認為CML是透過ROS/HIF-1α/TWIST路徑造成EMT,且以ALA前處理可以改善此現象,因此認為ALA可以作為預防和治療糖尿病腎臟纖維化的藥物。
