Abstract
口腔癌治療存活率在近年來治療方式進步下卻未有進展,其主要原因仍然是早期診斷的困難。當正常細胞轉型為癌細胞的過程中,因為基因表現改變,造成細胞週期失調、訊息傳遞異常及腫瘤血管新生,不論在核酸或蛋白質的質、量及轉譯後的修飾方面皆有其獨特的變化。
本實驗以質譜檢測技術分析比較在動物模式中誘發老鼠口腔癌發展之不同時期血清檢體其N-聯結醣基化蛋白表現圖譜,發現激肽原(Kininogen-1)等蛋白質表現於腫瘤早期發展過程中有明顯上升,再以此N-聯結醣基化蛋白標記於臨床口腔癌患者腫瘤病理切片上以免疫染色方法驗證及利用酵素免疫法分析臨床病患之血清檢體。最後利用實驗室中口腔癌腫瘤細胞株細胞於RNA、蛋白質體層次檢視激肽原等蛋白質之表現及以基因調控檢測此特殊蛋白質在腫瘤細胞株上之功能表現。
實驗中發現激肽原在切片中腫瘤組織部分的表現隨腫瘤大小及癌症分期(AJCC)有顯著的差異,並且影響治療初期(24個月)之存活率。而由實驗室中驗證在正常口腔上皮細胞株與口腔癌細胞株中激肽原蛋白質的表現確有變化且可能影響腫瘤細胞之侵犯、移動及血管新生能力。由本實驗可知激肽原不論在臨床早期診斷或作為腫瘤標靶治療之標的都是一具潛力的腫瘤生物標記。
