Abstract
目的:孤獨會影響老年人的身心健康,已成為公共衛生的重要議題。本研究目的為探討社會關係、健康的改變是否會影響老年人之孤獨感以及社會關係、健康狀況改變以及孤獨與生活滿意度之相關性。
方法:資料來源為臺灣中老年身心社會生活狀況長期追蹤調查的民國100年與104資料,選取55歲以上且兩波均完成訪問者,分析樣本為2512人。孤獨採用De Jong-Gierveld的六題短版量表測量,以100年因子和100-104年兩波間因子變化做為自變項,解釋104年的孤獨與生活滿意度情形。分析軟體使用SPSS軟體22.0,分析方法包括敘述統計、雙變項分析、與迴歸模式;中介變項分析則採用SPSS Process Macro進行。
結果:(1)孤獨感平均分數為3.86(SD=2.09),高於其他國家。(2)在控制基礎年自變項後,社團參與變無(β=0.350)、接受工具性支持變無(β=0.979)、負向支持變有(β= 0.271)、接受情緒性支持變少(β=0.294)、接受資訊支持減少(β=0.264)、經濟滿意度變低(β=0.334)、壓力變高(β=0.318)、憂鬱增加者(β=0.398)以及疾病數變多(β=0.182)與較高的孤獨程度有關;變為有社團參與(β=-0.345)、接受工具性支持變有(β=-1.498)、負向支持變無(β=-0.496)、自覺幫助程度變高(β=-0.253)、提供資訊性支持變高(β=-0.483)、經濟滿意度變高(β=-0.282)、壓力變低(β=-0.312)、以及憂鬱症狀變低(β=-0.438)與較低孤獨程度有關(R2=0.354)。(3)情緒性孤獨與社會連結、經濟滿意度、壓力和憂鬱症狀的改變有關(R2=0.151)。社會性孤獨感與社會連結、社會參與、接受工具性支持、接受情緒性支持、提供資訊性支持、負向支持、自覺幫助程度、經濟滿意度、壓力、憂鬱症狀和慢性疾病數的改變有關(R2=0.305)。(4)生活滿意度與較低的孤獨感相關(β=-0.164)。此外,當控制基礎年因素時,較高的生活滿意度與社會連結、接受工具性支持、負向支持、自覺幫助程度、接受情緒性支持、經濟滿意度、壓力、宗教信仰、憂鬱症狀和身體功能障礙的變化有關(R2=0.501)。
結論:老年人的社會支持、社會參與、經濟滿意度、壓力和憂鬱症狀的改變會影響其孤獨感和生活滿意度。較高的孤獨感與較低的生活滿意度相關,並且是社會關係和健康狀況變化與生活滿意度的中介因素。建議政策上應保障高齡者的經濟安全和協助建立社區內的社會連結網絡,以促進老年人的心理健康與福祉。
