Abstract
近年來,病人安全議題逐漸受到全球的重視,一連串的醫療疏失將醫療安全問題突顯出來,更直接衝擊醫病關係。院內輸血是醫療照護的重要一環,且是屬於高風險的醫療行為,對病人安全影響巨大,如何確保醫院輸血安全,防範輸血風險成為現今醫療界非常關鍵的問題,因此,本研究目的即為透過預應式的風險管理方法-醫療失效模式與效應分析來改善醫院輸血流程。

本研究首先使用問卷調查,欲了解個案醫院醫護人員對病人安全及風險管理的認知程度,另外應用醫療失效模式與效應分析(Healthcare Failure Mode and Effect Analysis, HFMEA)進行對醫院擇期手術中非預期輸血的流程改善,研究案例為某寧波市某醫院,進行方式包含資料收集,問題釐清、組成團隊、繪製流程圖、執行危害分析及擬定改善對策與執行行動與效果量測。

問卷調查結果顯示年紀在「21~25歲」及年資在「0-3年」的醫護人員對病人安全的認知較高。針對醫院擇期手術中非預期輸血流程,自2016年7月開始進行改善,將輸血流程分為五大部分及二十二個次流程,發現8個高風險流程,列出14項潛在失效模式、27個潛在原因。研究除探討潛在的危險因子外並加以實證改善,其中八項潛在失效模式透過加強培訓課程、輸血申請系統改善、設計並建立交接表單等方式,改善後有七項失效模式之危害指數達到50%以上的改善率,並且在改善後透過收集用血不良事件數以及計算用血等待時間,再用血等待時間的改善效果上,病人用血等待時間明顯較改善前縮短至30分鐘左右。

整體而言,本研究運用醫療失效模式與效應分析的方法,除了清楚探討整個輸血系統潛在風險並改善外,亦能透過改善達成品質管理要求,並增進病人安全,達到建構安全醫療體系的目標。
