Abstract
研究動機與目的
從1978年第一例試管嬰兒誕生至今,全世界各地接受試管療程的人數逐年上升;然而成功懷孕率仍然是無法令人滿意。成功懷孕始於胚胎成功的在子宮腔裡著床,而對於子宮內接受胚胎著床的機轉部份,在生殖醫學領域中還是一個未知的黑盒子。子宮內膜允許胚胎著床的能力(又稱子宮內膜容受性endometrial receptivity)受到各式基因及表基因調控完成,其中最穩定的是DNA甲基化調控。過往研究針對子宮內膜容受性研究絕大部分是透過侵入性子宮內膜切片完成;但是如果想要研究在同一個月經週期的內膜狀況,非侵入性的方式還是必要的。過往文獻可已知子宮頸分泌物裡面可以偵測到跟著床相關的細胞激素;另外從癌症的研究經驗發現可從子宮頸分泌物分離出游離DNA,其甲基化表現和原始病灶具有高度相關性;因此,本研究利用子宮頸分泌物DNA甲基化型態來當作研究子宮內膜容受性的非侵入性的替代方式,探討其與胚胎植入後的著床懷孕結果之相關性。

重要研究方法及研究結果
我們收集接受冷凍胚胎植入女性在植入當天的子宮頸分泌物,分析子宮頸分泌物DNA甲基化型態與懷孕與否的相關性,並利用生物資訊分析及機器學習來預測懷孕的正確率。研究結果發現,從文獻上篩選出候選基因的子宮頸分泌物DNA甲基化型態在有懷孕及沒懷孕的女性身上有顯著差異,另外導入不同機器學習的演算法可以得到最高86.67%的成功懷孕預測正確率。其次,使用全基因體DNA甲基化分析技術也證實子宮頸分泌物甲基化型態在有懷孕及沒懷孕的女性身上有顯著差異,藉此可得到正確預測懷孕率在所有檢體高達86%,而在有懷孕的檢體則有96.4%正確率。

結論
綜合上述結果顯示DNA甲基化型態和子宮內膜容受性相關,具體表現在有懷孕及沒懷孕的週期其DNA甲基化型態有顯著差異。我們的研究證實可以透過非侵入性的方式,利用子宮頸DNA甲基化型態來評估當週期子宮內膜容受性,如此可在臨床應用上做為一種非侵入性預測胚胎植入成功率之生物指標,幫助臨床醫師選擇較佳週期來施行胚胎植入,提升成功懷孕機率。
