Abstract
心臟病一直都是國人十大死因第二位,臨床上大約80~85%的心因性猝死(Sudden cardiac death, SCD)是心律不整所致。而Implantable cardioverter defibrillator (ICD)已經成為心因性猝死之預防最標準及有效的治療措施,但很少研究探究電擊當下造成的身體反應及誘發因素。本研究目的在探討ICD電擊的誘發因素及分析ICD電擊時的身心反應。
本研究採橫斷式以結構性問卷訪談方式收集資料,共收集75位個案,其中「有經歷過電擊」32位、「無經歷過電擊」43位。使用自行發展「ICD電擊誘發因素量表」及「ICD電擊身心反應量表」,測量電擊誘發因素及其身體與心理反應之頻次。
研究結果顯示,個案基本屬性與是否經歷電擊經驗無關,最常現電擊的誘發因素為「心跳加快/心悸」、「胸悶」、「暈眩」、「過度疲勞」、「中至強度運動時」及「喘」。而ICD置入時間與電擊誘發因素呈顯著正相關。電擊時心理反應大於身體反應,且身心反應前五項均為心理因素,分別為「我害怕下次何時再發生」、「我感到重獲新生」、「我感到病情惡化」及「我感到驚嚇(毫無預期)」及「我感到接近死亡」。
此外,「每年電擊總次數」、「裝置後電擊總數」及「電擊誘發因素」與身心反應總分、身體反應、心理反應均呈現正相關,但「電擊誘發因素」是唯一的預測變項。
依據上述,本研究「ICD電擊的誘發因素」及「ICD電擊時的身心反應」的結果,可運用於臨床醫護人員對ICD病患的了解並提昇照護品質。
