Abstract
肌少症(Sarcopenia)為常見的骨骼肌退化性疾病,隨年齡增長及慢性疾病發生率上升,導致肌肉質量與功能喪失。肌少症主要因肌肉纖維萎縮所致,尤其快肌纖維流失速度遠高於慢肌。本研究探討兩種非侵入性刺激之近紅外線(Near Infrared,NIR)與低強度脈衝超音波(Low-Intensity Pulsed Ultrasound, LIPUS)對肌少症的潛在治療益。
NIR為本實驗室前人研究所建立之條件(740 nm 波長、照射 35 秒),已證實能促進快肌比例提升。LIPUS 則經小鼠肌肉母細胞株C2C12實驗,以電壓 ± 20V、頻率2.5 MHz、每次5 分鐘、間隔1 分鐘、共重複三次刺激,亦可提升快肌表現。
動物實驗以7 週齡雄性斯普拉格-道利(Sprague-Dawley, SD)大鼠進行後肢懸吊模
型,以成功誘發脛前肌(tibialis anterior, TA)萎縮,並分別使用NIR、LIPUS 與兩者聯合(N-U)刺激,持續14 與28 天。然而透過行為測試(滾輪、曠野和抬趾)與TA 肌肉組織分析(TA 重量、免疫螢光和西方墨點法),評估治療效果。
結果顯示,NIR(250 mAs、波長740 nm、每日刺激35 秒)於短期與長期皆能穩定提升快肌纖維比例與運動能力,皆具有治療效果;LIPUS(±20V、2.5 MHz、每次5 分鐘、間隔1 分鐘、共三次)在長期刺激後對行為與快肌表現有部分改善,但相較於細胞實驗其成效有限,可能與穿透效率或能量分布不均有關;N-U 則呈現快肌纖維與行為能力逐步提升的趨勢,具長期治療潛力。整體而言,NIR為效果最穩定之單一治療方式,LIPUS 仍待優化參數,N-U 具進一步發展價值。三種非侵入性刺激方式皆對肌少症具正向之療效,未來若延長刺激時間,或許可進一步提升治療成效,並增加臨床應用潛力。
