Abstract
乳癌是全世界最盛行的癌症之一。根據台灣地區2007年的癌症登記資料,乳癌為女性癌症發生率的首位,其死亡率更高居女性癌症死因的第四位。以前研究指出乳癌的危險因子其可歸因百分比約為20~40%包括年齡、賀爾蒙相關危險因子、飲食、運動、體重或身體質量指數及遺傳易感受性等。因此尋找未知的乳癌危險因子成為乳癌研究的重點,人類乳突瘤病毒在近年被認為可能是乳癌的危險因子。在1992到2010年間共有18個病例系列研究與8個病例對照研究,研究顯示乳癌腫瘤組織中人類乳突瘤病毒的盛行率其範圍分別為0~68%與4.4~86.2%。這些證據顯示人類乳突瘤病毒可能參與乳癌發展過程。然而目前對於乳癌腫瘤組織中,是否存在人類乳突瘤病毒的研究有爭議。人類乳突瘤病毒感染與乳癌的相關性會隨樣本數多寡、HPV檢驗技術靈敏度的高低及地理區域的差異而改變。因此我們提出一個假說:乳癌的形成與HPV感染有關,與乳癌相關之HPV高危險型別不限於一般導致子宮頸癌的高危險群HPV型別。我們進行研究去檢驗這一假說。我們的結果顯示乳癌組織與纖維腺瘤之HPV陽性率為37.1%(23/62)與10.0%(2/20)。62例乳癌檢體其病人平均年齡為53.7±11.9年,最年輕是29歲,最年長是85歲。20例纖維腺瘤檢體其病人平均年齡為30.3±8.7年,最年輕是18歲,最年長是45歲。乳癌組織與纖維腺瘤石蠟組織檢體其HPV Blot檢測為陽性者,使用費歇爾精確檢定評估HPV感染導致乳癌發生的相關性之危險對比值與95%信賴區間。HPV感染與乳癌有關其危險對比值為5.3 (95% CI:1.1-20.5;P =0.026)。乳癌組織中測得HPV基因型的分佈依比率高低排列:HPV-33 (16.1%)、HPV-18 (6.5%)、HPV-58 (4.8%)、HPV-16 (3.2%)、HPV-52 (3.2%)、HPV-70 (3.2%)、HPV-72 (3.2%)、HPV-6 (1.6%)、HPV-11 (1.6%)、HPV-39 (1.6%)及HPV-42 (1.6%)。在組織免疫化學染色研究,比較病例組與對照組中生物標記與乳癌的相關性,Rb、syndecan-1及MDM2可顯著降低乳癌風險[Rb (OR, 0.4; 95% CI:0.1-1.1;P =0.26)、syndecan-1 (OR, 0.17; 95% CI:0.02-1.3;P =0.1) 及MDM2 (OR, 0.14; 95% CI:0.04-0.4;P =0.00)]。比較病例組與對照組中人類乳突瘤病毒與生物標記的相關性,研究顯示所有蛋白質的表現與HPV感染均沒有統計上的相關(P >0.05)。表示樣本數不足,仍須導入大規模的病例對照研究,以期能夠確認HPV感染導致乳癌發生的危險對比值。這些訊息將有用於研發乳癌相關之專一型別人類乳突瘤病毒疫苗以降低婦女的健康威脅。
