Abstract
背景:糖尿病為全球主要公共衛生挑戰之一,其慢性病程及併發症對個人健康、家庭與社會造成重大負擔。台灣自1996年起推動「糖尿病共同照護網」計畫,期望透過跨專業整合照護與標準化健康管理提升病患健康行為與生活品質。然而,現行政策評估方法多聚焦於經濟指標,較難衡量政策產生的多元社會價值。因此,本研究試圖引入社會投資報酬率(SROI)分析方法,探討其在台灣糖尿病照護政策評估上的可行性與應用潛力。
方法:本研究採SROI分析設計,依循《A Guide to Social Return on Investment》(2012年版)所提出的六大步驟,建構初步之評估架構。研究聚焦單一利害關係人(糖尿病患者),以「回診行為改變」為主要成果指標,分析期間為2017年1月至2019年1月。研究過程包括利害關係人辨識、改變理論與事件鏈建構、投入與產出項目推估,以及財務代理變數設定等,並以模擬資料與公開文獻進行估算。
結果:本研究聚焦於SROI分析方法的建構與應用流程初探。研究過程中完成了利害關係人辨識、成果鏈建構、投入與產出關係推估,並指出在變數設定與成果貨幣化過程中所面臨的挑戰與侷限。結果顯示,SROI方法有助於揭示現行評估方法較難量化的社會價值面向,具備作為健康政策分析輔助工具的潛力。
結論:SROI提供一種整合經濟與社會價值的評估視角,適用於慢性病管理與健康促進政策。透過本研究之初步分析流程建構,顯示SROI方法於台灣糖尿病共同照護政策具應用潛力,未來可進一步發展為具制度化與量化操作可能之健康政策評估工具。
