Abstract
研究背景與目的:
譫妄是加護病房常見的急性腦功能障礙,與住院時間延長、呼吸器使用天數增加、長期認知功能衰退及死亡率上升密切相關。然而,目前臨床常用之譫妄評估工具如CAM-ICU與ICDSC,在病人意識不清或需持續觀察時具一定侷限性。隨著自動紅外線瞳孔測量技術(Automated Infrared Pupillometry, AIP)應用於神經重症照護的普及,瞳孔參數作為生理性預測指標之潛力逐漸受到關注。本研究旨在探討自動紅外線瞳孔儀所測得之瞳孔參數是否能有效預測加護病房病人發生譫妄,並建立具臨床意義之預測模型。
研究方法:
本研究為單中心前瞻性觀察性研究,於台灣北部某醫學中心加護病房進行,收集157位成人重症病人資料。採用NeurOptics® NPi-300自動紅外線瞳孔儀每日早晚測量瞳孔參數,包括瞳孔光反射百分比變化(q‑PLR)、瞳孔收縮速度(CV)、瞳孔擴張速度(DV)、神經瞳孔指數(NPi),並依據加護病房譫妄篩查清單(ICDSC)進行譫妄診斷。個案入住加護病房急性生理及慢性健康評分 (APACHE II)分數、呼吸器、鎮痛劑累積劑量及查爾森共病指數等潛在與譫妄相關的參數資料也同時收集。分析方法包括t檢定、多變項邏輯斯迴歸與接收者操作特徵(ROC)曲線分析。
結果:
本研究樣本之譫妄發生率為64.3%。譫妄組之年齡、APACHE II分數、呼吸器使用率及鎮痛劑累積劑量顯著高於非譫妄組。首次瞳孔測量中,譫妄組之q‑PLR、CV、DV與NPi均顯著低於非譫妄組(p值皆<.05)。多變項邏輯斯迴歸分析顯示,在控制年齡、疾病嚴重度與藥物等干擾因子後,q‑PLR為具預測力之變項(OR=0.931,p=.007)。其ROC曲線下面積(AUC)為0.698,最佳切點為17.25%,具中度特異度(81.8%)與低度敏感度(50%)。
結論:
本研究發現,自動紅外線瞳孔儀所測得之q‑PLR可作為加護病房病人潛在譫妄風險之預測指標。未來可進一步整合其他生理與臨床變項,發展多元預測模型,以利早期辨識高風險個案,提升譫妄預防與照護效能。
