Abstract
背景
隨著精準醫療與人工智慧技術的快速發展,臨床決策支援系統(Clinical Decision Support Systems, CDSS)已成為提升診療品質與決策效率的重要工具。然而,現行多數 CDSS 在處理中文電子病歷及因應複雜癌症治療規則時,仍面臨語意理解不足、知識維護困難與可解釋性有限等挑戰。特別在肺癌這類需多元條件判斷的治療場景中,系統化建構與邏輯推論更顯困難。
方法
為解決上述問題,本研究提出一套整合自然語言處理、本體導向知識圖譜與可解釋推理模組的智慧型肺癌臨床決策支援系統。系統以南部某醫學中心實際肺癌個案為資料來源,運用多層次語法規則與語意主題建模將非結構化病歷轉化為結構化語意資訊。推理核心依據 NCCN 臨床指引與健保給付規定建構邏輯節點與規則鏈,並加入可追溯的推薦說明模組,以增進決策透明度與使用者信任度。
結論
實證結果顯示,系統整體推薦召回率為54.66%,其表現受限於病歷資料的結構化與完整性;但於基因變異子任務中表現卓越,一致率可達99%以上。系統推薦不全然與醫師選擇相符,顯示其能在遵循指引的同時提供具臨床依據的替代選項,增強決策靈活性。此系統亦驗證了先前應用於風濕性關節炎之推理架構能成功遷移至腫瘤治療領域,展現跨疾病應用潛力與擴充性。
此外,針對不同治療類型之推薦表現進行細部分析發現,系統於標靶藥物類別具備最高推薦精準(Precision值92.16%),顯示其對結構化基因資訊能進行有效對應與推論。化療部分則呈現較高召回率但推薦精確度偏低,可能與指引適用情境多元與非結構條件難以完全擷取有關。免疫藥物推薦則為目前系統表現最弱環節,其推薦精確度不足,反映出在 PD-L1 表現與免疫相關資訊擷取仍有待強化。
從總體指標來看,系統所有建議之總體 Precision值28.75%,顯示推薦中仍有部分藥物未被實際採納,建議策略仍需更精準貼合個別病人情境與臨床實務考量。未來應加強推薦條件的精緻化設定與模型對臨床差異性的辨識能力,以提升建議品質並降低誤推薦率。
本研究除探討資料品質與知識更新等挑戰外,亦提出多中心驗證、多模態整合與角色導向解釋等未來發展方向,為人工智慧於癌症臨床決策之實務應用提供具體參考與啟發。
