Abstract
醫院所儲存的病人電子病歷(Electronic Medical Record, EMR)結合醫院外的健康數據(Electronic Health Record, EHR),所形成的個人健康數據(Personal Health Record, PHR),已成為世界各國發展醫療人工智慧基礎和健康資訊發展的重要目標,病人可經由PHRs來掌握自我的健康訊息,但PHR推動上仍面臨許多問題待解決。近幾年,台灣健保署推動健康存摺應用程式,讓民眾跨院查詢相關的PHRs,但民眾參與意願不高,使用率偏低。

本研究主要目的將合作醫院所開發區塊鏈應用程式(iWellChain DApp),並整合健保署的健康存摺SDK,解決現有健康存摺的問題與限制。另一方面,因應COVID-19爆發期間,為了瞭解民眾對於PHRs是否具有行為上的改變,透過iWellChain DApp的訪問日誌,分析民眾研究期間內的活動情形。
最終,本研究成功地整合健保署的健康存摺SDK到iWellChain DApp當中,讓病患能夠定期查詢他們跨醫療院所的數據,特別是COVID-19篩檢以及疫苗接種相關訊息。另一方面,本研究分析iWellChain DApp使用者在2019 年 7 月至 2021 年 6 月期間的使用日誌數據。從 2020 年 1 月起,研究結果顯示申請iWellChain DApp下載並用其訪問醫療數據的用戶人數和使用iWellChain DApp查詢門急診紀錄和檢驗報告的人數都逐年增加。在所有訪問醫療數據中,其中查詢門診紀錄、門診檢驗報告、急診紀錄在2021年6月達到峰值,依序分別為34.61%、41.87%、52.40%,急診檢驗報告則在2021年5月達到88.10%。經過對用戶訪問日誌的分析顯示,發現這些峰值與2021年5 月至6月台灣當地的 COVID-19 爆發時間點相互吻合,並且COVID-19 期間大幅度增加了患者對 PHR 的使用意願以及他們預防 COVID-19 的健康意識。
