Abstract
研究目的:探討呼吸訓練對血液透析患者之心率變異數、焦慮及生活品質改善成效探討。
研究方法:本研究採叢聚隨機化試驗,將參與者區分為每週一、三、五透析者為第一組與每週二、四、六透析者為第二組,採用投擲硬幣的方式進行隨機分配實驗組和控制組,探討八週呼吸訓練對血液透析患者心率變異數、焦慮及生活品質改善成效。收案對象為台北地區某醫學中心,長期需接受血液透析治療患者,共獲得有效樣本119位(實驗組56位和控制組63位)。兩組前測皆須接受心率變異儀器監測、情境特質焦慮量表以及世界衛生組織(台灣簡明版)生活品質問卷測量,並個別教導呼吸訓練方式並說明呼吸訓練為每次透析期間每小時分別進行五次呼吸訓練,每週三次共持續八週。實驗組每週監測心率變異的變化,而控制組則在第四週監測一次心率變異數,於第八週再進行後測資料收集。所得資料以百分比、最大值、最小值、平均值、標準差、t-test、卡方檢定、two way ANOVA,進行統計分析。
研究結果:(一)八週呼吸訓練於不同時間點之比較,實驗組TP值後測高於前測,於統計學上有顯著差異(p<0.001),實驗組HF值後測高於前測在統計學上有顯著差異(p=0.042),實驗組生活品質後測得分高於前測,於統計學上有顯著差異(p<0.001),實驗組情境焦慮後測得分低於前測達顯著差異(p<0.001)。(二)二因子重複測量變異數分析進行統計處理,於組別與測量時間心率變異TP值(F=10.815,p=0.001)、心率變異HF值(F=7.547,p=0.007)、生活品質得分(F=104.548,p<0.001),情境焦慮得分(F=71.310,P<0.001),實驗組與控制組在時間與組別上達到相互作用的成效。(三)實驗組在八週呼吸訓練期間的每週檢測心率變異的變化,在不同調查期間之平均心率變異TP值(F=6.796,p=0.012)、HF值(F=5.951,p=0.018)皆呈現顯著性線性趨勢,表示心率變異LF值及HF值在八週呼吸訓練期間隨著時間有上升的趨勢。
研究結論:本研究結果得知血液透析患者接受八週呼吸訓練後,可有效提昇整體心率變異數,並增強副交感神經活性,降低情境焦慮程度以及改善生活品質。
