Abstract
背景
中風是全球殘疾調整生活年度的第二大原因,也是台灣第三大死因。目前中風的診斷由於缺乏合適,快速,準確和分析敏感的基於生物標誌物的測試而受到阻礙和延遲。轉運蛋白(TSPO)是以各種組織類型表達的外線粒體膜蛋白。其分佈根據疾病,疾病階段和病變的接近程度而變化。

目標
我們假設急性缺血性卒中(AIS)患者血清TSPO水平高於健康對照組。此外,入院TSPO將準確預測AIS患者的卒中嚴重程度,神經功能和出院生存。

材料和方法
兩個對照組是每個病例的年齡和性別匹配。病例是首次出現症狀發作48小時內進入卒中單位的患者。在入院和出院時評估實驗室和臨床參數。對照是沒有中風史或任何神經退行性疾病史的健康志願者。從所有研究的參與者獲得人口信息,生活方式參數和自我報告的疾病史。通過酶聯免疫吸附測定(ELISA)測量TSPO。通過Student t檢驗和Mann-Whitney U檢驗,探討了連續數據組之間的差異。卡方或Fisher精確檢驗用於分類變量。條件邏輯回歸用於研究高血清TSPO水平與AIS匹配設計之間的相關性。另外,繪製接收器工作特性曲線(ROC)分析,曲線下面積(AUC)。

結果
本研究納入了38例AIS患者和76例對照者。與對照組相比,AIS患者的血壓,葡萄糖,c反應蛋白,糖基化血紅蛋白,白細胞和肌酐水平顯著升高。對照組的高密度脂蛋白膽固醇平均血清水平顯著升高。中風患者血清TSPO水平高於健康對照組的5例(95%CI = 2.0-12.6,p <0.001)。此外,TSPO顯著區分了AIS患者與健康對照(AUC 0.839,截止值為0.284ng / mL,敏感性為78.1%,特異性為66.5%)。在AIS患者中,TSPO(臨界惡化0.479ng / mL)顯著地將臨床惡化(AUC 0.764,敏感性80%,特異性69.6%)和功能能力BI <30(AUC 0.751,靈敏度77.8%,特異性75%)差異顯著卸貨。具有較高入院TSPO水平的AIS患者的NIHSS入院與出院患者有正差異,而在TSPO水平較低的患者中觀察到陰性NIHSS差異。

結論
血清TSPO水平將AIS患者與健康對照區分開來。較高的入院血清TSPO水平是AIS患者出院時神經功能不良預後差的良好預測指標。然而,為了得出關於TSPO和AIS之間的聯繫的結論,有必要對具有不同人口和較大樣本量的更多研究。
