Abstract
乳癌為女性最常見且致死率高的惡性腫瘤之一,其中三陰性乳癌(Triple-Negative Breast Cancer, TNBC)因缺乏雌激素受體(estrogen receptor , ER)、黃體素受體(progesterone receptor , PR)及人類表皮生長因子受體2(human epidermal growth factor receptor 2 , HER2)的表現,使TNBC成為具有高度侵襲性與轉移性的乳癌類型,目前臨床上缺乏有效的治療方式與適當的分子標靶。天然物在抗癌藥物研發中扮演重要角色,特別是在提升療效與降低副作用方面具有發展潛能。Piper longum L.(長胡椒)富含多種具抗氧化、抗發炎與抗腫瘤活性的天然小分子化合物。當中 Piperlongumine (PL) 因其突出的抗腫瘤活性備受關注。在本研究中,透過天然小分子化合物Piperlongumin (PL) 的處理顯著提升TNBC細胞的抗腫瘤活性。研究結果中顯示,PL處理能有效抑制 TNBC 細胞的增殖並促進細胞凋亡,進而抑制腫瘤的遷移與侵襲能力。此外,PL處理能誘導 Hippo 訊號通路中AMPK以及LATS1的磷酸化,並有效抑制 Yes-associated protein 1(YAP)的表現。另一方面,我們在轉移性 TNBC 細胞的 RNA 定序分析發現當中表現差異的基因,並預測當中與腫瘤轉移及病人存活率相關的轉移風險因子。其中,我們發現在TNBC中所篩選出來的轉移風險因子內皮素-1(Endothelin-1, ET-1)與 YAP 的表現顯著相關,並與 TNBC 患者較差的預後有關。同時我們證實,ET-1 的表達受到 YAP 的轉錄調控,且 PL的處理能有效抑制YAP,並伴隨YAP調控的 ET-1 表現下降。最後在免疫組織化學分析中,我們發現ET-1 與 YAP 同時呈現上調表現,無論是在小鼠或是乳癌臨床組織切片的轉移性腫瘤組織當中,而 ET-1 的過度表現與 TNBC 患者的不良預後密切相關。本研究揭示了PL 透過抑制 YAP-ET-1 訊號通路來抑制TNBC細胞的增殖同時促進細胞凋亡,進而抑制腫瘤的遷移與侵襲能力。綜合以上所述,PL具備作為潛在化療藥物的潛力,在未來可成為 TNBC 治療策略中一項極具潛力的輔助劑,對 TNBC 患者提供新的治療方向。
