Abstract
背景
過去研究指出失智症(dementia)病人的Cathepsin D濃度比正常人低,Galectin-3濃度比正常人高。然而,在輕度認知功能障礙(Mild Cognitive Impairment, MCI)者的研究資料很少。因此,探討血液中Cathepsin D和Galectin-3在認知功能障礙者的表現是必要的。
目標
探討血漿 Cathepsin D 及 Galectin-3 濃度與認知功能障礙及認知功能惡化的相關性。
方法
從士林社區世代(2004迄今)中選取在2011和2016兩年皆有蒙特利爾智能量表(Montreal
Cognitive Assessment MoCA) 的社區60歲以上民眾,並可取用2011年問卷資料及血清檢體者,共計161人(男72人、女89人)。採用病例對照研究。並使用兩種設計來判斷病例及對照。 以2011年MoCA分數分組之橫斷性研究,以2011年時MoCA 分數低於26分,作為認知功能障礙組 (n=73),大於等於26,作為正常組(n=88)。另以2011至2016年間MoCA分數變化的追蹤型設計,將2011年、2016年的MoCA分數分別取標準化分數(Z score),取2016年Zscore減去2011年Z score後,作為5年變化值並分組,Z值差大於等於0分,定義為認知能力持平,為五年認知功能無惡化組(n=81),Z值小於0為認知能力下降的五年認知功能惡化組(n=80)。年齡、性別等社會學變項和糖尿病診斷等變項來自問卷,而其中糖化血紅蛋白(Glycated Hemoglobin,HbA1c)是用高效液相色譜法(High Performance Liquid Chromatography, HPLC)、血糖 (Glucose) 是使用酶法 (Enzymatic Assay)測定;CatD、神經膠質蛋白2 (Neuronal Pentraxin II,NPTX2) 、C反應蛋白(C-reactive protein, CRP)和磷酸化Tau蛋白181 (Phospho-Tau181,P181)是用使用酵素結合免疫吸附分析法(Enzyme-linked immunosorbent assay, ELISA) (ab119586,abcam,CatD) 來檢測。使用獨立樣本t檢定 (Independent Sample t test) 去比較輕度認知功能障礙組與對照組是否有顯著差異,使用邏輯式迴歸(Logistic regression)去得出的這兩組的OR值,使用卡方檢定 (Chi-Square Test) 去計算分組人數。
結果
在以2011年MoCA分數分組之橫斷性研究中,認知功能障礙組的Cat D為3380.06±353.11pg/mL,正常組為3486.57±253.80pg/mL,兩組間的Cat D濃度有顯著差異(P=0.003);認知功能障礙組的Galectin-3 (Gal-3)濃度顯著高於對照組(認知功能障礙組為72.23±35.51 pg/mL,正常組為59.99±24.80 pg/mL,p=0.015),而HbA1c、NPTX2、P181、Glucose和年齡性別等指標,除年齡(P= 0.013)以外皆沒有顯著差異(p>0.05)。而當CatD濃度以最低濃度區間(<3241.89 pg/mL)為參考組,僅有當CatD濃度在3465.22~3652.33 pg/mL之間時,有顯著保護性,勝算比為0.32(95% CI:0.13-0.80,p=0.015),以Gal-3 < 43.09 pg/mL為參考組,僅當Gal-3濃度 > 81.38 pg/mL時,其勝算比為2.609(95% CI:1.061-6.416,p=0.037),具有顯著的危險性。
在以MoCA分數變化探討血清標誌物差異之追蹤研究(以MoCA分數變化探討血清標誌物差異之追蹤研究)為追蹤型設計中,五年認知功能無惡化組的Cathepsin D為3508.2±257.61 pg/mL,五年認知功能惡化組則為3366.3±336.98 pg/mL,兩組間有顯著差異(P=0.003),而五年認知功能惡化組的收縮壓(SBP)顯著高於五年認知功能無惡化組(131.00 ± 18.60 mmHg vs. 123.91 ± 16.92 mmHg,p=0.012),而HbA1c、NPTX2、P181、Glucose、CRP、Gal-3和年齡性別等指標在兩組間皆沒有差異(p>0.05)。而當CatD濃度以最低濃度區間(<3241.89 pg/mL)為參考組,當CatD濃度在3465.22-3652.33 pg/mL及濃度高於3652.33 pg/mL時 0.212(95% CI:0.08-0.561,p=0.002)及0.344(95% CI:0.135-0.874,p=0.025),當SBP > 140 mmHg時,其勝算比為3.05(95% CI:1.296-7.177,p=0.011),具顯著風險。
結論
本研究揭露血液中的CatD可橫斷性地觀察到MCI(即MoCa低於26分)的關聯性,並可做為預測未來5年認知功能下降的生物標誌,而Gal-3也可橫斷性地觀察到MCI(即MoCa低於26分)的關聯性。
