Abstract
本研究藉由回溯性分析來了解影響白色念珠菌(Candida albicans)真菌尿症(funguria)治療的因子,作為日後臨床白色念珠菌的研究參考。收集北部某醫學中心自2007年4月~2008年9月,因白色念珠菌所造成的真菌尿症,接受fluconazole治療至少5天及有治療結果之個案並留有檢體者,納入此次研究計劃,治療結果成敗以開始治療後6天(含6天)後之尿液顯微鏡分析或培養做為評估,若尿液顯微鏡分析仍有膿尿(每個高倍視野中多於5個白血球)或培養白色念珠菌陽性則算治療失敗;若同一病人60日內有兩次(含兩次)白色念珠菌真菌尿症則是為同一個案。回朔性研究法調查發現:自2007年4月~2008年9月共375個白色念珠菌所造成的真菌尿症病人,其中收集65人次感染個案符合收案條件,經專人重新鑑定並調閱病歷收集相關資料,包含個案使用fluconazole的劑量、抽血檢驗數據與病人的潛在疾病及治療結果進行分析。結果顯示:65個案中治療成功的有28個案,失敗的有37個案。使用fluconazole的劑量與治療結果沒有相關,而血中肌肝酸(creatinine)大於2mg/dl、病人潛在有糖尿病、呼吸衰竭與尿毒症,達到統計學上的差異,p值<0.05,是治療失敗的危險因子。
