Abstract
乳癌經手術後,需合併術後的輔助性化學治療以避免復發或轉移的情形。先前研究指出以歐洲紫杉醇含蒽環黴素類藥物之治療處方相較於僅使用蒽環黴素類藥物或不含蒽環黴素類藥物等傳統治療處方,可顯著提高無病存活率和總存活率,但在藥物不良反應與購置成本上需花費較多成本,是否具成本效果仍有值得討論的價值。本研究的研究目的為探討乳癌化學治療藥物歐洲紫杉醇在健保給付前後,相較於傳統治療處方,使用於早期乳癌患者作為輔助化學治療的存活情形及直接醫療成本效果。
本研究為回溯性世代研究,資料來源為健保資料庫及癌症登記檔長表,研究對象是已進行手術之新發成年女性乳癌患者,觀察組為2007年至2010年使用歐洲紫杉醇含蒽環黴素類藥物之治療處方者,比較組為2002年至2005年使用傳統輔助化學治療處方者,兩組以傾向分數進行一比一配對,配對後各為1,652人,以標準化差異呈現配對前後共變項的平衡情形。以Kaplan-Meier存活函數估計、分層Cox比例風險迴歸模式與Nelson-Aalen累積風險函數估計分析兩組的總存活率、乳癌特異性存活率、無病存活率與乳癌復發率。成本效果分析以增加成本效果比、成本效果平面、信賴橢圓呈現。
兩組經傾向分數配對後,於個人特質、就醫醫院特性、乳癌相關臨床變項與乳癌相關治療等共變項皆呈現平衡。在追蹤一年及兩年之總存活率、乳癌特異性存活率與無病存活率,觀察組顯著高於比較組。而追蹤一年及兩年之乳癌復發率,觀察組顯著低於比較組。在直接醫療成本效果分析方面,以總存活天數為單位的兩年增加成本效果比為6,889元;以乳癌特異性存活天數為單位的兩年增加成本效果比為7,430元;以無病存活天數為單位的兩年增加成本效果比為3,678元;以無乳癌復發天數為單位的兩年增加成本效果比為4,652元,各增加成本效果比在成本效果平面上皆位於第一象限,顯示較具有成本效果。
本研究的結論為早期乳癌患者使用以歐洲紫杉醇為基礎之輔助化學治療處方,在短期的存活情形方面,總存活率、乳癌特異性存活率與無病存活率顯著高於傳統輔助化學治療處方、乳癌復發率顯著低於傳統輔助化學治療處方,在直接醫療成本效果分析方面相較於傳統治療處方較具有成本效果。
