Abstract
長期透析患者常出現蛋白質-熱量營養不良,相對於血液透析,腹膜透析因透析過程流失的胺基酸與蛋白質更多,患者更易出現營養不良與導致的相關問題,改善其營養狀況是臨床營養工作者的重要任務,常見方法包括營養教育與口服高蛋白補充品等。動物性的牛奶蛋白與植物性的大豆蛋白皆屬於高生物價的完全蛋白質,是透析患者飲食蛋白質的良好來源。然而研究顯示大豆蛋白比起動物性蛋白有更多的保健功效,包含改善血脂、延緩腎功能惡化,且植物性蛋白所含的磷吸收較低,對於透析患者可能有更正面的影響。本研究旨探討不同蛋白質來源-牛奶蛋白與大豆蛋白的補充,對腹膜透析患者營養狀態與相關代謝的影響。
本研究2015年9月至2016年12月於台北慈濟醫院及2017年10月至2018年2月於新光吳火獅紀念醫院腹膜透析門診進行。所有受試者皆接受營養師飲食指導,無意願規律服用營養品者收入對照組共計14位,願意規律服用營養品者隨機分組,共收入29位牛奶蛋白組與28位大豆蛋白質組,每日服用一包補充品(16.8g蛋白質);試驗期共12週,每4週追蹤飲食狀況與門診常規血液檢查。結果三組12週後皆顯著提升腹膜透析病人血清白蛋白、對造組血磷值顯著上升、牛奶蛋白組及對照組實驗中血清肌肝酸顯著上升,顯示經由營養教育或營養教育合併蛋白質補充品使用,有效提升腹膜透析患者進食狀態,進而改善其營養狀態;蛋白質補充品因額外鈣的添加,限制飲食磷吸收,進而協助患者血磷的控制。
