Abstract
背景:安眠藥的使用族群中,女性一向是多於男性,雖然zolpidem因其安全性被大量開立,然而,如何正確地使用zolpidem,遂成為包括精神醫學在內的各醫學專科皆需正視的問題。為保障民眾用藥安全,中央健保局於2012年11月6日函請各特約院所,並於102年2月進入輔導改善期,凡開立Nimetazepam、Flunitrazepam、Zolpidem等三項安眠鎮靜藥品,務必進入「保險對象特定醫療資訊查詢平台」進行查詢,查詢就醫對象近期就醫用藥行為與數量。理論上,此期間之不當處方可定義為故意為之。本研究將分析「保險對象特定醫療資訊查詢平台」成立前後,台灣女性族群Zolpidem不當處方的相關因素分析與比較。
目的:利用健保資料庫分析台灣女性族群之Zolpidem不當處方(inappropriate prescriptions)的相關因素。
研究方法:以2000年承保資料檔中擷取資料,研究期間為2012年1月1日至2013年12月31日。本研究利用全民健保資料庫之「門診處方及治療明細檔(CD檔)」及「門診處方醫令明細檔(OO檔)」,篩選出「Zolpidem處方之女性病患」後,定義該次處方是否屬於不當處方,並進一步分析各個自變項與「Zolpidem不適當處方」之相關性。
結果:本研究共計篩選出167,182人次服用zolpidem的女性患者。在病人特質的部分,女性患者zolpidem不當處方的相關因素為病人的科別、社經地位與城鄉差距,低社經地位與農業鄉鎮的女性最易出現zolpidem不當處方;在醫院特質部分,相關因素則有醫院的層級與權屬別,就診於公立醫院與診所的女性最易發生zopidem不當處方;在醫師特質部分,相關因素則是醫師的年齡、專科別與專科年資,醫師年齡越低越易開立女性zolpidem之不當處方,而精神科是最常發生女性zopidem不當處方的科別。
結論:醫師開立zopdiem時應更加謹慎,醫護人員也應更積極提供衛教;公立醫院與診所將是政策上改善女性zoldidem不當處方的重點項目;台灣的精神科不當處方浮濫,應加強稽核,並強化心理諮商師的介入。
