Abstract
世界醫學教育聯合會(World Federation for Medical Education, WFME)所提出的醫學教育全球標準包含基礎醫學教育、畢業後醫學教育及醫師之繼續專業培育。所有的醫學教育需包含「醫學預科」、「基礎醫學」、「臨床醫學訓練」這三個階段,在臨床醫學訓練方面,需先進入UGY(Under Graduate Year)、PGY(Post Graduate Year)最後才到專科訓練。對於醫學生而言,於大二下才開始接觸基礎醫學教育課程,所以在尚未進入醫學教育前,我們先給予初階臨床體驗課程,讓醫學生在未來行醫的道路上能以「病人」為中心,而非以「病」為中心。
本論文以臺北醫學大學所舉辦的初階臨床體驗課程為研究對象,針對臺北醫學大學及三家附屬醫院(北醫附設醫院、萬芳醫院、雙和醫院)進行分析。量化研究評估方式採用三種學習動機(Learning motivation),包含了自我效能、內在價值及考試焦慮三個面向來進行探討,想了解初階臨床體驗課程對於醫學生基礎學科成績的關連性。質化研究分為兩種方式,第一部分以初階臨床體驗課程的學生撰寫雙週札記為文本,依據五個面向(「影響專科選擇」、「學習態度的改變」、「提升醫學知識、態度、技能」、「紓緩學生壓力」、「能協助塑造專業角色」)進行分析,了解參與初階臨床體驗課程的學生在過程中產生的變化;第二部分為初階臨床教師的訪談,採用立意抽樣及深度訪談方式進行,藉由訪談了解參與初階臨床體驗課程的學生與學堂老師之間的互動狀況,及學堂老師對學生的影響進行探討與分析。質與量分析都為學堂學生與老師之間的關係,也就是「師徒制」所產生的影響。
本論文有四個發現:
(1)動機信念會影響基礎學科課程有顯著的影響
(2)初階臨床體驗課程學堂對於基礎學科課程有顯著的影響
(3)初階臨床體驗課程對學生醫學知識的影響存在關聯性
(4)透過師徒制學生與老師之間的關係和動機信念存在關聯性
