Abstract
研究背景
保栓通 (clopidogrel)與鈣離子阻斷劑有藥理的交互作用存在,造成clopidogrel 藥效減少,心血管栓塞風險增加。由於亞洲族群對clopidogrel 代謝與歐美族群不同,且在台灣,因急性冠心症且執行經皮冠狀動脈介入治療的族群,使用clopidogrel與鈣離子阻斷劑是常見的治療處方。目前僅有兩篇以丹麥保險資料庫數據發表有關clopidogrel與鈣離子阻斷劑與執行經皮冠狀動脈介入治療的文獻,結果是沒有顯著差異的。本研究以台灣健保資料庫,探討clopidogrel與鈣離子阻斷劑,在符合健保給付規定,clopidogrel與鈣離子阻斷劑是否存在臨床交互作用,因而增加心血管栓塞事件。
研究目的
本研究目的是分析2001年1月至2010年12月,在半年內診斷為急性冠心症且第一次執行經皮冠狀動脈介入治療的病患,同時使用保栓通與鈣離子阻斷劑,是否真實存在交互作用,導致保栓通藥效減少而增加心血管阻塞事件,導致再次住院再次執行冠狀動脈介入治療。
研究方法
本研究為回溯性研究,以2001年1月到2010年12月健保資料庫,收錄在半年內診斷為急性冠心症,第一次執行經皮冠狀動脈介入治療的病患,排除執行冠狀動脈繞道手術、或中風且使用保栓通、或沒有執行冠狀動脈介入治療的病患。組別分為使用無暴露組、保栓通組、鈣離子組與同時使用保栓通與鈣離子組共4組。本研究利用Incidence Rate與Cox 比例風險模式比較保栓通與鈣離子阻斷劑的交互作用,導致心血管阻塞事件的風險。
研究結果
本研究納入27558人,分為無暴露組 (N=10425)、Clo組 (N=6822)、CCBs組 (N=6120)與併用組 (N=4191) 兩組。整體的倂用組對照無暴露組的風險為2.44,對照CCBs的風險為3.73,對照Clo組的風險為0.61。整體以CCBs組風險對低,倂用組風險次之,Clo組風險最高,都有顯著差異。
結論
保栓通併用CCBs不會存在臨床交互作用,使用CCBs可減少心血管阻塞事件的風險,雖然不知真正的保護機轉為何,需再進一步分析。
