Abstract
胃癌病人的存活率與早期診斷及有效治療息息相關,所以搜尋早期胃癌生物標記及發展專一性標靶藥物為當務之急,本研究目的便是利用蛋白質體二維微差電泳分析技術搜尋具潛力之胃癌組織蛋白生物標記,並且針對獲得的生物標記發展胃癌診斷或治療藥物。從臨床胃癌組織檢體中,經與同個體鄰近正常組織比較分析後,我們共發現三個具顯著差異(p<0.05)的胃癌生物標記,包括大量表現的glucose regulated protein 78 (GRP78)及glutathione s-transferase pi (GSTpi),與下降表現的alpha-1 antitrypsin (A1AT),大量表現的GRP78在此研究中被使用作為標靶蛋白用於傳送診斷或治療藥物,以達到更有效及專一性的藥物作用。我們的結果顯示依據WIFPWIQL序列設計的GRP78結合胜肽(GRP78 binding peptide,GRP78BP)於體外可選擇性的辨認及結合胃癌MKN45細胞株,並且於腫瘤鼠中增進高分子聚合物micelles所包埋的藥物進入癌細胞並達到較佳療效,另外我們也設計並製備GRP78BP導引micelles用於核醫造影診斷腫瘤,GRP78BP首先與Mal-PEG-PCL鍵結,再與鍵結diethylenetriamine penta-acetate (DTPA)的PEG-PCL混合形成GRP78BP-micelles-DTPA,利用DTPA鍵結放射性同位素indium (In)-111應用於腫瘤核醫造影,以instant thin layer chromatography (ITLC)分析標誌效率顯示DTPA-micelles及GRP78BP-micelles-DTPA 與In-111的標誌效率分別測定為85%及93%,利用nanoSPECT/CT進行放射性腫瘤造影以追蹤GRP78BP促進高分子micelles聚集腫瘤效果,結果顯示GRP78BP引導的micelles- In-111 與單獨micelles- In-111比較於腫瘤組織具有較高的放射性訊號,證實GRP78BP可幫助 micelles 聚集於腫瘤組織使訊號提升約2倍 (p<0.05)。利用GRP78BP增進高分子micelles用於腫瘤標靶治療的策略或許可提升此類藥物精準攻擊腫瘤,並且降低因化療所造成的體內副作用,我們的結果證實GRP78是胃癌標靶診斷或輔助治療的生物標記,可應用於核醫腫瘤診斷並提升腫瘤治療,此新穎GRP78BP引導的micelles聚合物或許可應用於臨床輔助傳統腫瘤組織切片診斷及化學治療。
