Abstract
研究背景:
台灣地區人口老化比例正逐年攀升,老年人常見的慢性病膝部骨性關節炎,其發生率與盛行率均隨著年齡而增加,膝部骨性關節炎對老年生活品質影響甚鉅,對醫療成本的花費更是一大衝擊;依據統計,55歲以上的人口約有45%-68%的人罹患退化性膝關節炎,而75歲以上的族群則有65%-80%的好發率,且女性罹患退化性膝關節炎比男性又多了一倍,臨床上治療退化性關節炎病人可分為兩大類:一、傳統性療法,二、另類療法;玻尿酸屬於另類療法其中之一。隨著人口老化,執行關節置換術的手術量不斷的上升,相對的在醫療費用上也逐漸上漲,根據行政院衛生署2008年統計,每年接受全膝關節置換術的病患超過一萬人次,主要原因為退化性關節炎,其中老年人口占90%以上。

研究目的:
本研究探討退化性關節炎患者注射玻尿酸與人工膝關節置換之相關性。
採用回溯性研究探討退化性關節炎患者注射玻尿酸與退化性關節炎患者沒有注射玻尿酸,對於人工膝關節置換手術之相關性。

研究方法:
本研究使用全民健保資料庫篩選符合入選條件之對象,首先從資料庫2000.01.01~2006.12.31為止,篩選在這段期間內符合退化性關節炎門診一次或住院一次 (ICD-9-CM:715)之病患,去除資料異常,接著串聯相關檔案。每位退化性關節炎病患追蹤5年的醫療紀錄,探討其發生膝關節置換的概況。

研究結果:
本研究結果顯示施打玻尿酸會增加膝關節置換的風險,但其實這是健保局的規定加上健保資料庫的限制導致的統計誤差,由於退化性膝關節炎的病患關節內注射玻尿酸健保給付規定的限制,本研究使用全民健保資料庫做為資料來源,挑選出來的使用玻尿酸組本身就是退化性關節炎比較嚴重的病患,既然較嚴重那之後發生膝關節置換的風險當然顯著的高於沒有施打玻尿酸的患者。
依據年齡、性別的不同退化性關節炎其發生率皆有顯著的差異,相對於女性病患膝關節置換風險顯著高於男性病患,由於停經後賀爾蒙流失進而導致女性多半有骨質疏鬆的狀況,退化性關節炎在45 歲之前男女發生率差不多,但45 歲以後女性一旦進入更年期發生率及明顯高於男性。並且年齡每增加一歲,膝關節置換風險也顯著增加。
