Abstract
研究背景與目的:
隨著癌症發生率與存活率上升,社會對於腫瘤護理師個人和專業有著高度要求。面臨各種照護上的壓力,同時需要在道德倫理複雜的情況下執行決策。高度壓力的工作環境容易影響腫瘤護理師心理健康,進而導致離職率上升。而復原力,是護理師在高壓的醫療環境下自我應變、調整、恢復,維持工作的耐受性的能力,但較少研究關於腫瘤科護理師的復原力。本研究在探討護理人員心理健康狀態、復原力與離職傾向之相關性,進而了解該如何增強腫瘤科護理師復原力,維持其心理健康,減少腫瘤護理師之流失。
研究設計:
研究對象為癌症專科醫院之臨床腫瘤護理師,採橫斷性問卷調查,使用情緒自評量表、醫療人員復原力量表及離職傾向量表作為研究工具,共計收案177人。
研究結果:
32.6%的腫瘤護理師有輕度到嚴重程度的憂鬱情形,39.5%有焦慮情形,35.6%壓力高於正常範圍。憂鬱、焦慮及憂鬱的影響因子有加班時間、睡眠時間及規律運動。心理健康與離職傾向呈現正相關,並且與復原力呈現負相關。腫瘤護理師復原力偏高平均為39.48分,上過復原力或壓力因應課程之護理師復原力較高且離職傾向較低。男性、骨髓移植病房及有加班情形之腫瘤護理師,較容易出現離職想法。而憂鬱程度可預測離職想法之風險,輕度至非常嚴重組相較於正常組其離職想法顯著高出5.8倍。
結論:
雖然腫瘤護理師整體復原力偏高,但仍有三分之一護理師出現憂鬱、焦慮、壓力等心理問題。心理問題與離職傾向相關,且憂鬱可顯著預測離職想法,顯示欲想降低離職傾向,就需要減輕護理師之憂鬱。機構可以加強心理健康篩檢,提出政策優化職場環境,減輕心理健康問題,以減少腫瘤護理師之流失。
