Abstract
研究背景
運用新穎的機器學習技術有望預測和管理與萬古黴素和太古盤寧素相關的急性腎損傷。先前的預測模型受限於單一時間點的數據,限制了臨床的應用性。將時間序列數據和可解釋的人工智慧技術融入到機器學習中,有助於在多個時間點進行預測,並提高模型的解釋性。

研究目的
本研究旨在開發並驗證適用於臨床的機器學習模型,用於預測萬古黴素和太古盤寧素相關急性腎損傷。

研究方法
這項回顧性研究使用臺北醫學大學臨床研究資料庫,以2010年至2020年間接受靜脈注射萬古黴素或太古盤寧素的患者為研究對象。訓練數據集包括來自萬芳醫院和雙和醫院的80%數據,用於模型的開發,而餘下的20%則用於內部驗證。外部驗證則採用臺北醫學大學附設醫院的數據集。患者數據以天為單位收集,包括基本資料、入院診斷、共病症、用藥紀錄和實驗室檢查結果。為了優化兩種機器學習算法(極限梯度提升和輕梯度提升)的特徵集,本研究運用遞歸特徵消除法結合特徵重要性或SHAP (SHapley Additive exPlanations)重要性。模型表現以準確率、精確率、召回率、特異性、陰性預測值、F1分數、接收者操作特徵曲線下面積(AUROC)和精確率-召回率曲線下面積(AUPRC)進行評估。最後利用SHAP解釋模型,並根據F1分數選擇最佳模型的最優閾值。

研究結果
本研究納入了9342名使用萬古黴素或太古盤寧素的患者。其中,在訓練集中有19.6%的患者(1374/7020)、在內部驗證中有19.1%的患者(335/1755),以及在外部驗證中有20.5%的患者(116/567)發生急性腎損傷。透過使用SHAP特徵重要性的遞歸特徵消除方法,極限梯度提升模型找出一個包含95個特徵的精簡特徵集,內部驗證顯示其在AUROC和AUPRC方面表現優越,分別達到0.806(95% CI 0.795-0.818)和0.238(95% CI 0.218-0.258),外部驗證則為0.800(95% CI 0.781-0.817)和0.266(95% CI 0.232-0.300)。SHAP分析顯示,使用萬古黴素或太古盤寧素的天數越少、尿素氮和總膽紅素濃度越高,以及同時使用越多種腎毒性藥物都會增加預測萬古黴素和太古盤寧素相關急性腎損傷的機率。在調整閾值為0.1後,F1分數達到最大值,以增強模型在現實情境中的臨床應用性。

結論
可解釋的機器學習模型利用時間序列數據,能夠在多個時間點預測與萬古黴素和太古盤寧素相關的急性腎損傷,並協助進行個別風險評估和臨床決策。
