Abstract
背景:父母親全產期的心理健康影響著自身的生理壓力及子代後續的發展,當人體經歷壓力時,可能會刺激自身的下丘腦-垂體-腎上腺(HPA)軸調節,導致 HPA 軸的失調,皮質醇(Cortisol)可說是 HPA 軸調節後的產物,頭髮皮質醇濃度(Hair cortisol concentration,HCC)已被證實為可以有效反映出慢性生理壓力的生物標誌物,雖然過去已有許多探討父母親的心理健康與幼兒後續的發展的相關研究,但目前使用頭髮皮質醇作為生理壓力探討心理健康如何影響自身、學齡前兒童壓力賀爾蒙及學齡前兒童發展的相關研究甚少。

研究目的:本研究目的為(一)探討雙親心理健康、主觀壓力與自身皮質醇濃度的關係,(二)探討雙親心理健康、主觀壓力與學齡前兒童皮質醇濃度的關係,(三)探討雙親心理健康、主觀壓力與學齡前兒童發展的關係及(四)探討雙親心理健康、主觀壓力是否透過學齡前兒童皮質醇濃度影響其發展情形。

方法:本研究在大台北地區共五間醫院(醫學中心、教學醫院共五間)的婦產科門診邀請婦女及其伴侶參加研究,於懷孕初期至產後四年調查父母親雙親於孕產期間憂鬱、焦慮、幸福感,以及親職壓力等狀態,與學齡前兒童之發展情形,同時也蒐集父母親及兒童的頭髮樣本,最終共有131個家庭完成了問卷調查與提供了從懷孕初期到產後四年的頭髮樣本。其中有68個家庭被追蹤至產後時期並提供了學齡前兒童的發展情形結果,使用了多元迴歸(Multiple regression)進行結果分析。

結果:本研究發現父親的幸福感與父親的頭髮皮質醇濃度呈顯著負相關(r=-0.49,p<0.05),母親的焦慮(r=0.21,p<0.05)和感知壓力(r=0.39,p<0.05)與母親的頭髮皮質醇濃度呈顯著正相關。此外,母親的頭髮皮質醇濃度與學齡前兒童的頭髮皮質醇濃度(β=0.34,95%CI=0.05-0.53)以及學齡前兒童懷疑發展遲緩的風險較高(OR =3.25;95%CI=1.02-10.3)有關。

結論:父母心理健康較差與母親和父親的皮質醇水平較高有關。此外,母親的頭髮皮質醇與學齡前兒童的頭髮皮質醇以及懷疑發展落後的較高風險有關。因此,我們建議在懷孕期間進行早期篩查和健康教育計畫,以減少父母壓力並降低學齡前兒童發展遲緩的風險。
