Abstract
研究背景與目的
結核病為世界上僅次於COVID-19的傳染病殺手,其嚴重度不容小覷,世界衛生組織致力於在2035年能消除結核病。另外,糖尿病身為全球前十大死因之一,且研究指出糖尿病對免疫造成影響,進而增加結核病之發病率及治療成效不佳之風險,兩疾病之雙重負擔為公共衛生重大問題。Statins除了作用在HMG-CoA 還原酶來降低膽固醇生合成外,也被證明有其他多重功效,認為其有潛力調節免疫而降低結核病風險,然而目前對於statins在糖尿病患中是否能降低結核病風險文獻證據結果不一致。本研究欲改善先前研究設計降低偏差,探討降血脂藥品statins的使用對於糖尿病患者罹患結核病的風險影響。
研究方法
本研究為回溯性研究,使用衛生福利部資料科學中心提供的兩百萬人抽樣檔進行研究,分析2001年1月1日至2014年12月31日新診斷為非癌症之糖尿病患者,篩選出有使用statins的病人並分成三組cohort,分別為有使用statins的全體、排除掉特定免疫疾病者、以及免疫力較差者。各cohort再依據proportion of days covered(PDC)進行分組,將PDC為80%以上設為實驗組,20%至80%為對照組。各別之實驗組與對照組依照年齡、性別、干擾因子等計算傾向分數進行配對,並固定病患追蹤五年為試驗終點。以Cox比例風險回歸模型估計各study cohort中得到結核病和肺結核風險比,同時依累積用藥天數、累積用藥劑量和平均累積用藥劑量進行結核病風險分析。
研究結果
Cohort 1及cohort 2中實驗組和對照組相比,statins的使用會顯著降低結核病風險(aHR, 0.605 ; 95% CI, 0.42-0.871、aHR, 0.605 ; 95% CI, 0.404-0.905);cohort 3中有較低風險但無顯著差異(aHR, 0.678 ; 95% CI, 0.386-1.188);在肺結核分析中結果與結核病相似。在三個cohort中使用statins超過三年皆會顯著降低結核病風險;劑量分析在cohort 1及cohort 2顯示在平均低劑量或總累積高劑量下會降低風險。
結論
糖尿病患者使用statins可以降低發生結核病風險,statins在長期規律低劑量使用能夠顯著下降結核病風險。
